I understand I am being videotaped for the above-referenced video being produced for
AARG Action.

I hereby grant to AARG Action the right to take and use photographs, audio recordings
and video recordings of me; to use any or all of such photographs, audio recordings and video
recordings on websites and in communications, advertising, telecasts, literature, publications and
other communications in any form whether printed, graphic, audio or audiovisual, and in any
medium whether written, electronic or otherwise; to distribute, reproduce, copy, use and modify
such photographs, audio recordings and video recordings in any manner and by any method,
anywhere in the world, for any purpose, in perpetuity, and to grant to any other person or entity
the right to distribute, copy, and such photographs, audio recordings and video recordings in any
manner and by any method, anywhere in the world, for any purpose, in perpetuity; and to use and
display my name and likeness in or on, or in connection with the dissemination or distribution of,
such photographs, audio recordings and video recordings.

I understand and agree that I will not be entitled to any compensation for my appearance
in the video.

I hereby release AARG Action, its employees, directors, officers, agents and consultants
from and against any and all claims, demands and causes of action of every kind and nature,
which I may now have or hereafter acquire, arising out of or in connection with my appearance
in the subject photographs, audio recordings and video recordings; my participation in their
production and use, including, without limitation, any claims, demands and causes of action for
invasion of privacy or publicity, defamation, infliction of emotional distress and any other tort in
connection therewith; and any claims for any payment or royalties of any kind. I hereby waive
any right to inspect or approve the photographs, audio recordings and/or video recordings.

I represent and warrant that I am at least eighteen (18) years of age and am legally
competent to sign this permission and release form.
