GENERAL INSTRUCTIONS / REMINDERS

1. Write only those that have proper and complete proofs/documentations as shown in your portfolio.
2. Always refer to your portfolio for the correctness of your data.
3. Only those included in your portfolio that which are given equivalent units.

For A Credits - Educational Attainment


1. Arrange the entries with the latest school/university attended written first.
2. Spell out the name of the school/university, course and address. Do not use abbreviations/acronyms.
3. Input the units earned based on your Transcript of Records (TOR).

For B Credits - Educational Attainment


For Conference / Seminar / Workshop / Special Course / Technical Training Attended or Conducted
1. Arrange with the latest seminar/training attended or conducted written first.
2. Spell out the venue/location. Do not use abbreviations/acronyms.
3. Date should be written as dd/mmm/yyy. For example… 25-Dec-2009
4. Indicate the host/s that conducted or organized the activity. Do not use abbreviations/acronyms.
