Release Waiver and Quitclaim
I, ______________________, of legal age, Filipino, single, and with residential at

________________________, after having been duly sworn to in accordance with law, do hereby state
and undertake as follows:
1. I am the Lessee of the property located at _______________________ under the Contract of

Lease dated ________________ for a period of Three Years starting from ___________;
2. The period of lease ended on _________ and was no longer renewed;
3. I hereby acknowledge receipt _____________________________, the amount remaining from

the Security Deposit and which sums constitutes the full, entire and final satisfaction of whatever
interest or claim that I now or hereafter may have against the lessor __________, arising from or
in connection with the lease of the abovementioned property;
4. I voluntarily, irrevocably, unconditionally fully and forever waive, release and discharge the
lessor _____________, its successor-in-interest, directors, officers, stockholders, consultants,
employees and agents, past or present, from any and all claims, demands, damages, liabilities,
and compensation of every kind and nature whatsoever, suits, actions, and any and all causes of
action, either known or unknown, which in law and equity that I ever had, now have or may
have, in relation to, arising from, or in connection with, wholly, partially, directly or indirectly,
in the property and all transactions related thereto and all matters already covered by this
Release, Waiver and Quitclaim;
5. I confirm that this Release, Waiver and Quitclaim shall be binding upon myself as well as my
heirs, assigns, successors, representatives and/or persons acting under my direction, authority
and right, and that this Release, Waiver and Quitclaim may be pleaded as a bar to any actions,
suits, proceedings, or inquiries, which I and/or my heirs, assigns, successors, assigns,
representatives and/or persons acting under my direction, authority and right, may bring in any
court, office or agency of whatever jurisdiction, relating to, arising from or in connection with,
all the matters contemplated above;
6. In the event any of my heirs, assigns, successors, representatives and/or persons acting under my
direction, authority and right, will make such claims or institute suits and actions, contrary to this
Release, Waiver and Quitclaim, I and on behalf of such heirs, assigns, successors, assigns,
representatives and/or persons acting under my direction, authority and right, to immediately
cause the withdrawal of such claims or termination of such suits and actions, and to indemnify,
and to hold free and harmless, ___________, his successor-in-interest, directors, officers,
stockholders, consultants, employees and agents, past or present, from any and all expenses and
liabilities whatsoever;
7. I undertake not to disclose, either directly or indirectly, to any person all matters, discussions,
and transactions related to and covered by this Release, Waiver and Quitclaim, including, but not
limited to its existence, contents and execution and any and all information I learned from
_______________ in the course of the events that led to this Release, Waiver and Quitclaim. I
understand that this extends to my representatives, counsel, heirs, successors and/or assigns. I
understand that any breach of violation of this provision will cause damage to _______________
in an amount that is impossible to compute and ascertain with certainty. In lieu of actual
damages, I do understand that I could be held liable under the law for breach hereof.
8. I fully understand this Release, Waiver and Quitclaim, have personally read it, and have
voluntarily signed it with full and complete understanding of its meaning and intent and my
rights under existing laws.
