    AFFIDAVIT OF UNDERTAKING
    I, CELSO P. CASTIGADOR, of legal age, Filipino, married and a resident of Brgy. Sto.
    Nino Sur, Arevalo, Iloilo City, Philippines, after having been duly sworn to in accordance with
    law, hereby depose and state:

    1.) That I am a registered firearm owner/holder describe below:

    2.) That the above-mentioned firearms is listed in the Firearms and Explosives Office of
    the Philippine National Police Headquarters located in Camp Crame, Quezon City,
    Metro Manila, Philipppines;

    3.) That on May 03, 2012, while I was in my house, I found out that my licensed firearm
    which I kept inside my room was missing;

    4.) That on May 10, 2012 at 8:10am in the morning, I reported this matter with the Iloilo
    City Police Station 6 at Arevalo, Iloilo City and with a blotter report no. 2012-0650;

    5.) That despite serious efforts to locate the said licensed firearm, it cannot be found;

    6.) That I certify and confirm that the said firearm was neither confiscated, relinquished
    in favor of the government or sold to any private individual;

    7.) That I am requesting that the said firearm be declared a loose firearm under R.A.
    No. 10591 or otherwise known as the “Comprehensive Firearms and Ammunition
    Regulation Act” as it was lost/stolen in my possession without my knowledge;

    8.) That I am executing this Affidavit of Undertaking to attest to the truth of the foregoing
    and for whatever legal purpose it may serve.
