1. Introduction

A) ENTRY OF PERSONNELS:

1) Client to issue gate pass to each personnel working in DSM ENGINEERING


PLASTICS PEB PREMISES for TSEIL.
2) TSEIL will ensure Medical, Induction and Photo ID card- (Optional to Client).
3) Re-induction and medical if there is 3 month break- (Optional to Client).
4) Entry only with Safety shoes, Helmet and proper dress (dhoti, loose clothing not
allowed).
5) All electrical tools, welding m/c., lifting tools and tackles allowed entry and usage
only after certification by Client. Revalidation required every month for electrical
equipment and tools.

B) WORK:

1) Use of Personal Protective Equipment (PPE) compulsory.


2) All jobs to start only after valid work permit, authorised by designated Area In-
charge.
3) Daily Tool-Box talk to every worker before start on job.
4) Work above 1.8 Mtr. With double hook safety belt, duly anchored.
5) All jobs with adequate Supervision.
6) High risk/high hazard jobs should have 100% supervision from both TSEIL and
Client.
7) No High risk/high hazard jobs after sunset.
8) Start High risk/high hazard jobs only after risk assessment and approval by safety
officer.
9) Only crawling on beams. No walking.
10) Use Lifeline and double hook safety belt, Fall arrrestor etc. as per Risk assessment.
11) Only metal railing (not rope) around openings on floor/ground, trenches, building
edges.

C) ELECTRICAL:

1) Portable electrical tools should be 110v or less. 230 Volt portable tools are
Allowed only after proper authorization.
2) All Electrical jobs to be done by only Electrician with valid License.
3) Only D C Welding sets allowed into the site. A.C. welding set is not permitted
entry.
4) ELCB’s for all electrical tools and equipment including welding set, hand tools etc.
5) ELCB should be tested before every start with Test P.B and with the tester every 3
months.
6) Rotary/moving parts of all equipment should have safety guards.
7) All construction power cable to be laid above ground and not on the ground.
D) TOOLS, TACKLES:

1) All lifting tools & tackles, Crane/Hydra etc. should have valid test
Certificate.
2) Ladders-securing/anchoring facility, all rungs in place. Bamboo ladders not
allowed.
3) Daily check of Safety belts, Scaffolding, Electrical Tools.

4) All scaffolding must be metallic. Scaffolding by only competent.


5) Fire extinguishers near Gas cylinders, DG sets etc. First Aid box at site
office.
6) Gas cylinders, upright and fastened in trolley.

E) PERSONNEL:

1) Supervisor – minimum 2 year experience. Entry only after evaluation and


approval.
2) Nominal Safety officer(s) from day one
3) High skill (e.g. welder) workman should have valid certificate and pass test
before start of work.
4) All Statutory regulations should be strictly followed including Labour
license, Migratory labour license, ESI / P.F., Muster roll, minimum wage,
payment before 7th of every month etc.
5) No smoking, No intake of alcohol inside the site
6) Drivers of vehicles, cranes, Hydra etc. should have valid license
7) Emergency vehicle with driver during all the time when any workmen at
site
8) Compliance to Access control procedure

F) SYSTEM:

1) All the mandatory tests and checks should be duly documented for
verification by Client.
2) Updated Inventory of all electrical tools, welding sets & lifting tools.
3) Immediate information to Safety Officer on any accident. Written report
within an hour.
4) All vehicle movement as per the standard (fitness, speed limit, hand brake,
helper etc.)
5) Violation of any rule liable for penalty including suspension of
work/contract.

F) REFERENCE FOR FULL DETAILS:

1) HSE Standard on Safe working at heights.


2) Mandatory safety Guidelines on Electric Arc welding.
3) Procedure for entry into confined space.

2. GENERAL SAFETY SYSTEM / INSTRUCTIONS FOR THE


SUBCONTRACTORS.

Site Induction
All personnel’s shall be given a site induction before they commence work on site. The
induction shall comprise information on the various hazards which they may come into

contact with, use of PPE’s, first-aid locations, welfare facilities, instructions on the site
emergency procedures, warming and alarm systems, access routes and project specific rules
as stated by the client / TSEIL Engineer.

Induction registers to be maintained giving details of personnel induction.

Drug and alcohol


The possession or use of illicit drugs and alcohol on site is not permitted. Anyone who is
found possessing or suspected to be, under the influence of either will be removed from the
site.

Passes and uniforms


All contractors’ personnel should be issued project specific pass showing the skilled levels.
There should be uniform for sheeting gang and erection gang and if possible of two
separate colors.

Site supervision
The contractor must ensure that an employee of suitable seniority and authority, with
responsibility for Safety is always present on site during the course of the works, to
supervise and direct the works and to receive and implement instructions from Client /
TSEIL representative.

Statutory regulations
It is the duty of the contractor to have knowledge of all relevant legislation and take
account of it in the planning and execution of the work on project site.

Personal protective Equipment / Clothing


All personnel on a project site must wear a minimum safety helmet, safety goggles and
safety shoes. Additional personal protective equipment may be required dependent on the

tasks being undertaken or as directed by the risk assessment. Safety harness with double
hook is mandatory for all works above 1.8 m height.

Fire extinguishers are required with buckets and stand in places of hazardous substances
like paint storage, insulation storage and wherever hot work is carried out.

Test certificates
The contractor shall provide a list of equipment and vehicles to safety perform the work.
Cranes, hoists, slings, lifting tackles and other lifting equipment shall be selected as per
load carrying capacities. All hoisting and lifting equipment shall be thoroughly examined
by a competetent person as per client’s requirement or as per applicable laws and
regulations.

E.g. for Hydra and crane


- Load test certificate
- Insurance
- Certificate of registration
- RTO log book etc.

Test certificate for tools and tackles like:


- D-Shackles
- Flat web slings
- Wire slings etc.

First aid

The contractor shall provide that they are provided with, such equipments and facilities as
are adequate and appropriate in the circumstances for enabling first-aid to be rendered to
any of their employees if they are injured or become ill at work.

Infrastructure drawing
The contractor shall submit infrastructure drawing showing details like Stores, insulation
shed, D. G. Shed, Hot rolled and cold rolled stacking area, canteen/first-aid facility etc.

Toolbox Talks
The contractor shall conduct toolbox talks for all the workers daily. These toolbox talks
should include topics related to ongoing work activities and precautions for those works.

Warning signs, barricades and signals


Barricades shall be used wherever necessary for the physical protection of people or
property.

4. SAFETY PRECAUTIONS DURING MECHANICAL ERECTION

1. Adequate supervisory surveillance shall be available all the time. Before commencing any erection
work, the concerned supervisor must review the entire operation from the consideration of safety.

2. Materials / equipments should be properly loaded considering their weight, dimension, capacity of
the carrier, center of gravity of the load, clearance required for safety, etc.

3. While unloading structural steel from trailers by hydra / crane, adequate precautions should be taken
to make the trailers stationary by putting scotch on wheels and no body shall stand under hanging /
swing loads.

4. Clear passages should be left for easy handling and transportation of structures.

5. Ensure personnel in surrounding area are warned / preferably cordoned before the activity starts.
Only authorized personnel shall be allowed within the protected barricade area.

6. The persons engaged in materials handling shall always use safety shoes, helmet and hand gloves.

7. Slings, wire ropes, chains, lifting tackles, lifting machines etc. must be checked by competent person
before use.

8. The max permissible wear on the hook is 10% of its original height at the lead point.

9. The load should be checked for brake holding and impactness of the hook such that there is no
slipping of sling on hooks.

10. The sling should be secured firmly / preferably locked on the hook such that there is no slipping of
sling on hook.

11. Only one person trained in signaling will give signals.

12. All riggers, winch operators, crane operators etc. should know standard signals thoroughly.

13. No man in any case should be allowed to go under hanging / lifting load.

14. The boom or any part of the crane should be clear from any live electric line / service line.

15. Only one object should be lifted at one time. The object should be balanced.

16. Unloaded materials must be checked for stability at ground level.

17. No one should ride on load carried by crane.

18. No one should stand under the boom.

19. The load should be lifted smoothly without jerks.

20. Before starting the crane, the operator should ensure the brakes; limit switches and other emergency
switches are in perfect working condition.

21. Only trained and authorized operators will operate the lifting tackles.

22. The crane should not be left with lead in hanging position.

23. The load should not be allowed to swing the crane. Should be moved only when load is balanced and
not swinging.

24. No worker may begin work at his working place before understanding safety rules and instructions.

25. Work place should be kept clean and in good order.

26. All the ladders must be in good condition and well supported to prevent fall of persons.

27. While lifting and moving the loads, the hooks as well as the rope should be vertical.

28. No material / load should be dragged with the help of crane.

29. No crane should be used in weather likely to endanger its stability.

30. For heavy / hazardous erection jobs on the construction site, persons concerned should make
‘PROTOCOL’ of safe working and monitor for implementation.

31. Only those persons who are skilled in working at height should be engaged for jobs at height.

32. Written clearance should be taken from appropriate authority while working near gas line/ electrical
line etc. A safety protocol must be prepared before starting the job in hazardous area.

33. Guy ropes should be used while handling heavy materials.

34. Ropes and slings should be protected by providing over sharp edges.

35. Slinging should be done properly to prevent the load from slipping

36. Hand made slings must not be used.

37. Manila ropes or fibre ropes must not be used for lifting structures and equipment

38. The hands or fingers should be kept out of pinch point while hooking, slinging or unhooking.

39. Use of safety belt and ensuring of trying of line is a must for trying of pulley blocks or releasing of
slings from loads or fixing of sling loads at heights.

5. MEHTHOD OF STATEMENT FOR “WORKING AT HEIGHT”

1. Deployment of knowledgeable, competent, safety conscious supervisory personnel at site.


2. Fabricated structures / materials / equipments shall be brought to site mechanically by hydra, if not
manually.
3. Ground condition / movement of the crane / hydra shall be checked before starting the work.
4. Suitable packing (timber pieces, gunny bags etc.) around the structure / material / equipment shall be
used to prevent damage of slings and wire ropes during their use.
5. Getting available approved work permit.
6. After erection of the columns / beams, bolts and nuts and washers shall be tightened.
7. After erection of two columns, tie beams shall be fixed.

8. Hand tools (portable tools), nuts, bolts, washers are to be carried in the bags.
9. Skilled riggers shall be deployed on the job.
10. After erection of structure, scaffolding shall be made around the column for the welding.
11. Access / arrangement for riggers to untie the hooks / slings (at height) are confirmed.

6. CHECK-LIST

1. Ensure personnel in surrounding area are warned / preferably cordoned before the YES/NO
activity starts.
2. Ladder (when used) shall be secured YES/NO
3. Safety PPE’s shall be worn by all workmen (working at height) and anchored to a YES/NO
sufficiently strong rigid point / life line.
4. Hard hat (safety helmet) shall be worn with chinstrap by everyone. YES/NO
5. Proper working platform shall be provided with effective hand railing / toe-guard. YES/NO
6. All unwanted materials should be worn. YES/NO
7. Shoes / gloves shall be worn YES/NO
