4. Closes the door and curtains, or provides bed screen to the
patient’s room.
5. Adjusts to a comfortable height. Lowers the side rails on the
side of the bed from which the nurse is assisting the client.
6. Places or assists the client into an appropriate starting
position.
7. Evaluates frequency of repositioning as necessary.
8. Washes hands.
9. Documents position used and schedule per agency protocol.

a. Assists the client to assume horizontal or supine position.
b. Moves the client’s inside arm next to the client’s body with
hands against hip.
c. Rolls the client onto the stomach using shoulder and hip as
key point of control.
d. Turns the head to one side. Pillows may be placed under
the trunk.
e. Places the client’s arm at the side.
f. Allows the feet to hang over the mattress or supports with a
pillow high enough to keep the toes from touching the bed.
