POSITIONING AND DRAPING

ERECT / STANDING /UPRIGHT POSITION

a. Assists the patient to stand with either slippers on or with
bare feet on a piece of paper
b. Unties the gown and leave the uppermost tape. Folds back
the gown over the shoulders towards the front.
c. Allows the patient to walk in order to observe the gait.

SUPINE POSITION / HORIZONTAL RECUMBENT POSITION / DORSAL POSITION

a. Places the patient on their back. Places the drape sheet.
Covers the patient from the shoulder to foot part with
sheets hanging on the sides.
b. Places the hands either on the sides or flexed at the side of
the body.
c. Places a shallow pillow or folded towel under the patient’s
head and abdomen for support.
d. Places additional pillow under the lower leg.
e. Places the side rails up.
f. Returns the bed to the appropriate height. Place a chair
near the bed of the patient.

DORSAL RECUMMBENT POSITION

a. Assists the patient to lie on his back or in supine position.
b. Brings the patient at the edge of the bed.
c. Separates the legs, rotates them outward, and flexes the
knees so that the soles of the feet are flat on bed. Places
the arms either above the head or the hands on the chest.
d. Places the draping sheet diagonally on the patient so that
the opposite corners cover the legs. Folds back the top
corner over the chest.
e. Wraps the corner on the right side around the right foot.
Does the same with the left foot.
f. Folds the lower corner of the sheet above the abdomen.

DORSAL LITHOTOMY POSITION

a. Assists the patient in supine position.
b. Inserts legging or stockinet’s.
c. Adjusts the stirrups according to the size of the patient.
d. Slightly flexes the knees and thighs deeply towards the
abdomen. Elevates the lower legs and supports them with
stirrups.
e. Draws down the buttocks to the folding edge of the table.
f. Raises the arms above the head or flex the hands on the
chest.

TRENDELENBURG POSITION

a. Assists the client in a horizontal recumbent position.
b. Elevates the foot of the bed so that the lower trunk is higher
than the head and shoulder.
- Places several pillow on the foot part then place the
lower extremities on the pillow
- Places the foot part of the bed on the block.
c. Supports the shoulders and knees.
d. Drapes same as horizontal recumbent.

FOWLER’S POSITION

a. Places the client in a horizontal recumbent position.
b. Elevates the head of the bed to an approximate angle with
hips even with the middle of the bed.
c. Places a small pillow at the back, under the knee, under
arms, and under the head of the client.
d. Drapes same as horizontal recumbent.
SIDE – LYING POSITION / LATERAL POSITION

Moving from Supine to Side-lying

a. Slides hand underneath the client. Moves the client to one


side of the bed by lifting the client’s body toward the nurse
by:
1. The trunk
2. Lower trunk
3. The legs
Does not drag the client across the sheet.
b. Rolls the patient to a side-lying position by placing the
client’s inside arm next to the client’s body with palm of the
hand against the hip.
c. Crosses the client’s outside arm and logrolls the client’s
outside shoulder and hip.

Maintaining a Side-Lying Position

a. Places an additional pillow to support the top leg, side leg,


fully and equally supporting the thigh, knee, ankle, and foot.
b. Moves the lower arm forward slightly at the shoulder and
bend the elbow for comfort.
c. Places a pillow behind the back.

SIMS POSITION / SEMI – PRONE POSITION

a. Assists patient on supine position.
b. Assists patient to their side preferably on the left with body
inclined forward.
c. Extends the left arm behind the back and flexes the elbow of
the right arm forward.
d. Flexes the right thigh towards the abdomen with the knees
drawn up higher than the left knee which is slightly flexed.
e. Lays out the draping sheet as in horizontal recumbent.
f. Folds back drape on the other side to expose the part to be
examined.

PRONE POSITION
Performance points 1 2 3 4 CI’s Remarks
a. Assists the client to assume horizontal or supine position.
b. Moves the client’s inside arm next to the client’s body with
hands against hip.
c. Rolls the client onto the stomach using shoulder and hip as
key point of control.
d. Turns the head to one side. Pillows may be placed under
the trunk.
e. Places the client’s arm at the side.
f. Allows the feet to hang over the mattress or supports with a
pillow high enough to keep the toes from touching the bed.

KNEE CHEST

a. Places the patient in prone position.
b. Assists the client to kneel with knees slightly separated
c. Assists the patient bend forward letting the chest rest on the
bed.
d. Turns the client’s head to one side and place the client’s arm
either above the head or flexes the elbow and let it rest
alongside of the client’s body.
e. Drapes as in dorsal lithotomic. Drapes the patient properly
so that the area to be examined is exposed.
