    Submit PDS, DTR and accomplishment
    report of ACT-Botolan for the Period Nov.
    16-30, 2021.
    Transmit Training’s attendance sheet and Certificate of
    Appearance
    Billing statement of the caterer.

    Comply the lacking Documents of RFR
    (Masterlist w/ Certification, Bond
    Confirmation, RFR Slip, ECPP, Potability
    Cert., Microbiological Test, Drawings and
    Technical Plans).
    DECEMBER 2, 2021 Facilitate ACT Meeting and Re-iterate to
    the Team that dishonesty in preparing
    DTR can be ground for termination, also
    re-discuss the timeline in submission of
    DTR, Accomplishment Report and TE
    reimbursement. Discuss also the open Logbook
    folder system for those Barangay that will
    be requesting 2nd Tranche RFR.
    Follow up Confirmation Letter to those
    Barangay’s that have already posted their
    Bond Premium for KALAHI SP.

    timeline in the implementation of Phase 1
    Sub-Projects and the deadline of
    submission of DTR and TE Logbook
    reimbursement and their complete
    supporting documents on the 2 Newly
    Hired CEF’s assigned in Botolan ,
    Zambales
    Folow up Confirmation Letter of Subic
    Zambales regarding Bond Premium

    DECEMBER 6, 2021 Attended New Employee Orientation
    (NEO) Batch 3 thru Google meet.
    Distribution of signed SPA to CEF’s for Logbook
    notary.
    Follow up BLGU Counterpart and status
    of Bond Premium thru Phone call.
    DECEMBER 7, 2021 Attended 2nd day of New Employee
    Orientation (NEO) Batch 3 thru Google
    meet.

    documents.

    regarding the MLGU Counterpart.

    Maintenance formation conducted @ Certificate of
    Brgy. Mambog and facilitated by CDO IV Appearance
    Aladin Naje, assist the CV’s in preparing Locator Slip
    O&M Plan.
    DECEMBER 15, 2021 Follow up status of posting of Bond
    Premium and BLGU Counterpart.
    Attended Meeting with Brgy. Captains of Logbook
    Subic Zambales and MSWDO Mam Certificate of
    Nellie Pagar, re-orientation about the Appearance
    implementation of CFW. RSO
    Attended virtual meeting conducted by
    CPO mam Rose regarding the conduct of
    upcoming Pre Procurement Conference
