    A near miss incident occurred when a façade glass being replaced by a gondola broke and fell into the gondola. The bottom of the glass struck the gondola's handrail and electrical panel as it was being lowered by glass suckers, causing a portion to break off. No one was injured. An investigation found that additional securing of the glass and protective pads on the gondola were not used. A field control briefing had also not been conducted to review hazards and controls with the team. Steps will be taken to provide protective pads on the gondola and additional securing of glass going forward.

    Incident Brief

    • A Near miss incident was reported when façade double sided glass hit with gondola (Rope Suspended Platform -
    RSP) resulting bottom portion (Ailand) of glass broken and fell into the Gondola which was in the same level
    (level 4) while replacing the damaged Façade glass.

    • A team was engaged to replace the damaged façade glass in East side at level-4 with help of Gondola. Four
    person including supervisor were engaged at level-4 and three persons were on Gondola (one operator and 2
    fitter). Before the incident, the external de-glazing of glass done and completed internal de-glazing as well with
    bottom section.

    • While leaning the damaged façade glass toward the Gondola with help of glass suckers a portion of bottom site
    strike with handrail, and electrical panel box of gondola resulting a portion (Ailand) of glass broken and fell into
    the Gondola.

    Hight of façade window – 1800mm,

    Width – 1200mm,

    Weight – Approx 140 Kgs,

    Please note when the Gondola was working at the podium level demarked fatal zone was available.

    No injury was occurred to anyone. Incident Date: 27th Mar-2023

     Time : 16.40 Hours

     Location: DAIS Project, Level-4 (East Side)

    Cause Of Incident ( Underlying)

     Additional securing with masking tape or securing tape of glass was not implemented.

     Caution pads on the rope suspended platform handrails, electrical box were not provided to prevent the glass strike

    and broken from it.

     FCB to the concern crew was not conducted regarding hazards associated with the task and implemented controls.

     Poor handling/replacing technic of damaged façade glass.

     The risk was not identified by the sub-contractor team as they were lowering the damaged façade glass towards

    Gondola which was having handrails and electrical DB top of it.

     The take -5 process was a failure as the risk was not identified and mitigated in this case never came before as

    more then 40 Nos galls were replaced.
