    !, CRESENCIO DE ASAS, Filipino, of legal age, and currently residing at Barangay


    Cagogobngan, Catubig, Northern Samar, after having been sworn according to law, hereby
    depose and state that:

    1. I am the only living sibling of the late +DELFIN DE ASAS who died on JULY 26,
    2011 and was buried at ETERNAL GARDENS MEMORIAL PARK CORPORATION;

    2. I am willing and giving my full consent to CYNTHIA RUZ to process the exhumation
    and reburial of my brother;

    3. I am also willing and giving my full consent to ETERNAL GARDENS MEMORIAL


    PARK CORP. to EXHUME AND CREMATE (IF EXHUMED WITH FLESH) the
    remains of the late +DELFIN DE ASAS lot specifically described as LOVE (LAWN)
    SECTION: H BLOCK: 059 LOT: 3 and rebury at the same lot;

    4. The deceased DELFIN DE ASAS died single and childless;

    5. I, hereby hold ETERNAL GARDENS MEMORIAL PARK CORP., its stockholders,


    directors, officers, and employees free and harmless from any and all such liabilities,
    damages or claims should there be any on the said exhumation and reburial of the
    remains of the late +DELFIN DE ASAS.

    and reburial of my brother;

    3. I, hereby hold ETERNAL GARDENS MEMORIAL PARK CORP., its stockholders,


    directors, officers, and employees free and harmless from any and all such liabilities,
    damages or claims should there be any on the said exhumation and reburial.

    !, CRESENCIO ENCARNADO DE ASAS, Filipino, of legal age, and currently residing at


    Barangay Cagogobngan, Catubig, Northern Samar, after having been sworn according to
    law, hereby depose and state that:

    1. I am the only living sibling of the late DELFIN ENCARNADO DE ASAS (now
    deceased), who is the owner/holder of Certificate of Ownership No. 17420 (22045),
    issued by the Eternal Gardens Memorial Park Corporation;

    2. My said brother passed away without contracting any marriage, nor he had children
    with any women; as such, he had no immediate family except us who is his niece
    and that he lived with my younger sister Carina De Asas Nungay (now deceased)
    when he was still alive;

    3. As such, this is to certify that the Certificate of Ownership No. 17420 (22045) issued
    by Eternal Gardens Memorial Park Corporation which he had acquired during his
    lifetime was lost and/or misplaced;

    4. I have exerted my best diligent effort in locating the same but to no avail;

    5. I have executed this affidavit to attest to the truth and veracity of the following facts to
    inform the proper authorities concerned and for whatever legal intents and purposes
    it may serve.
