AFFIDAVIT OF LEGAL CAPACITY
I, LEAH MAE P. ARCAYOS-TANK, of legal age, married, Filipino, and
residing at Lot 4, Block 8, Ford St. Rosario Homes subdivision, Brgy. Dao, Pagadian City, Zamboanga del Sur after having been duly sworn to in accordance with law, do hereby state and depose that:
1. I was born on 6 March 1983 in Aurora, Zamboanga del Sur, Philippines;
2. I am a natural born citizen of the Republic of the Philippines;
3. Prior to my marriage to KARAN TANK on 11 December 2019, I have
never been married or entered into any marriage ceremony either in the Philippines or abroad and have not undergone any process of annulment, declaration of nullity of marriage or legal separation in the Philippines or abroad;
4. Since birth my status remains SINGLE and otherwise legally free to marry, hence, I was able to marry KARAN TANK as I have the legal capacity to do so;
5. I am executing this Affidavit for the purpose of informing the authorities
concerned of the veracity of the foregoing asseverations and for whatever legal purposes it may serve.
