Greetings!
We, the students at Davao Oriental State University under the Bachelor of Secondary Education
major in Mathematics program, wishes to conduct a study entitled “LEARNING DIFFICULTIES IN
POLYNOMIALS AMONG GRADE 10 STUDENTS IN MATI NATIONAL COMPREHENSIVE HIGH
SCHOOL”.
This study is primarily focus on the learning difficulties in polynomials among grade 10 students
through diagnostic assessment in Mati National Comprehensive High School. There are 16 sections
of Grade 10 students. These selected sections will represent the entire population of the school.
We humbly ask you to please answer each question as honestly and accurately as possible. This
survey is completely anonymous, and the researchers guarantee confidentiality of the data. If you are
uncomfortable answering any questions, you can leave that question blank.
Your participation is voluntary. Your refusal to participate will involve no penalty or loss of benefits to
which you are otherwise entitled. You may withdraw your consent at any time and discontinue
participation without penalty.
This survey asks for your age, gender, and sections. However, your response will be kept under
anonymity and confidentiality.
