    I, MARIA CLARITA CORTEZ and CLARISSA CORTEZ, both Filipino citizen,


    of legal age, and with residence address at Talon, Amadeo, Cavite, after having duly sworn in
    accordance with law, hereby depose and state:
    THAT –

    1. We personally know AUREA DE LUMBAN VILLANUEVA;


    2. We declare and certify that to the best of our knowledge he/she was born JULY
    03, 1942 in the town of TALON, AMADEO, in the province of CAVITE, in the
    Philippines;
    3. We are not related by consanguinity or affinity to the child’s parents;
    4. We are executing this affidavit to attest to the truth and veracity of the
    foregoing facts and for whatever legal purpose it may serve.

    1. That the real name of my wife when she was living is AUREA DE
    LUMBAN VILLANUEVA, am known by the name “STEPHANIE
    LABONITE“ and have used the same in my personal and official records
    and interactions, as shown by the following (a copy of which is hereto
    attached as Annex “A”);

    AFFIDAVIT OF CORRECTION

    I, __________________, of legal age, Filipino and a resident of Cebu City, Philippines after
    having been duly sworn to in accordance with law do hereby deposes and says that:
    1. I am the brother of FELI PINAS who died on
    _________________________________, a copy of his death certificate is
    hereto attached as Annex “A”;

    2. I am also the one processing his death benefit and funeral claims before the
    Social Security System;

    3. One of the requirements for the death benefit claim is to provide the certificate
    of live birth or in the absence thereof a baptismal certificate of the deceased
    member;

    4. My brother FELI PINAS did not have a certificate of live birth so I am going to
    submit a copy of his baptismal certificate issued;

    5. However, I discovered that the date of birth appearing in his baptismal certificate was
    erroneously recorded as March 31, 1987 which was different from the date of birth
    found in the government records;

    6. The Tax Identification Number (TIN) ID of FELI PINAS issued by the BIR shows that
    the date of birth is recorded as March 21, 1987, hereto attached is a copy of his TIN ID
    marked as Annex “B”;

    7. Hence, due to mere clerical or typographical error on the date of birth of FELI PINAS
    in his baptismal certificate, I would like to ask for the correction of his birth date to
    March 21, 1987 which is the same date found on the government records;

    8.That I am executing this Affidavit to attest to the truth of the foregoing facts and to
    whatever legal purpose this may serve.
