Subject: Slipping Incident at 11F Male CR CEU Makati Legazpi Village Campus
Dear Dr. Carlito,
I am writing this letter to narrate the incident happened earlier at 11th Floor Male CR and the
events at the hospital after. Please see the chronology of events below:
07:33 – As I was waiting for my Nutrition and Diet Therapy subject to start (scheduled 07:00 AM
– 01:00 PM every Monday and Wednesday under Mr. Ronald Aaron Akio), I went to the male
comfort room to urinate. I was with my three male classmates that time – Mr. Magat, Mr.
Fontanilla and Mr. Lumayno.
07:35 – As I was going to the sink to wash my hands, I slipped on a water ponding in front of the
sink. As a result of the slipping, my head hit the sink which in turn resulted to me suddenly
sitting down on the CR floor. My head was profusely bleeding (specifically on the top of my left
eyebrow). My three classmates then assisted me to lay down on the CR floor. They also went to
get cotton to clean the blood from my wound. Somebody also immediately called the clinic
nurse.
07:40 – The clinic nurse arrived and assisted me to get on a wheelchair then we proceeded to
the clinic at 2nd Floor where he continued to clean and dress my wound. The clinic nurse also
went to get my vital signs (temperature and blood pressure). He also simultaneously called my
mom to inform what happened.
07:45 – The clinic nurse got a hold of my mom. He instructed my mom to fetch me from the
school with the intention of my mom would be the one to bring me to a hospital.
08:30 – My brother arrived at the clinic (2nd floor). The clinic nurse and other staff instructed us
to wait for the van provided by CEU. The van would be the one to transport us to out preferred
hospital. While waiting, the clinic nurse filled my brother information on what happened and
what will we be doing once we arrive on the hospital. Also, my brother went to see the male CR
at 11th Floor. My brother saw the area was quickly cleaned and a trash bin was placed in front of
the sink.
08:49 – The van arrived and proceeded to transport me, the clinic nurse and my brother to Rizal
Medical Center at Pasig City.
09:35 – We arrived at the emergency room at Rizal Medical Center. The clinic nurse escorted us
to the Triage. After settling in and right before registration, the clinic nurse instructed us to
update him and left.
09:45 – At the Rizal Medical Center, the triage doctor then endorsed me to the surgery unit of
the ER.
09:50 – The doctor and interns at the surgery unit of the ER interviewed and assessed me then
endorsed me to have my head undergo x-ray. They also instructed my brother to buy tetanus
vaccine and nylon from the nearest drugstore.
11:00 – The doctor proceeded to a skin test and inject the tetanus vaccine on both arms.
11:30 – Skull X-Ray complete.
14:00 – A doctor examined and tested my vision to initially check if the eyes were affected by
the incident.
15:30 – The doctor proceeded to stitch my wound. Procedure lasted for
16:45 – An ophthalmologist further checked if my eyes were affected. No damage was found on
the eyes.
17:00 – As per the doctor at the surgery unit, no fractures were found from the x-ray result.
Surgery department proceeded to process our papers for clearance. A follow up check up was
scheduled on December 18, 2023 at 11 AM on the Surgery Unit of the Out Patient Department
of Rizal Medical Center. And we were also given prescription on antibiotics and paracetamol. We
then proceeded to payment.
Attached herewith are the receipts of the vaccine, nylon, antibiotics and paracetamol. Also
attached are the receipts on the hospital charges.
