UNDERSTANDING THE SELF

SELF-ANALYSIS FORMAT 2023


Guidelines as you write your self-analysis:
1. Font: Arial or Times New Roman, or as long as the font is readable.
2. Font Size: 11 (for the content)
3. Line Spacing: 1.5
4. Paper Size: 8.5” x 11” (letter)
5. Orientation: Portrait
6. Margins: Moderate (check layout)
7. With CONFIDENTIAL Watermark
Guidelines in submission:
1. Minimum of 8 pages and maximum of 10 pages
2. Every part must be in order as described in the format given.
3. You can put pictures in each section for description
4. Should be submitted in PDF format in your iStudy account.
5. You can be as creative as you want.

PART 4: PERSONAL HISTORY
 Write in paragraph all experiences related to yourself or your life (you can put pictures in this
section for rich description of your personal history)

A. Developmental History (place of birth, place where you grow, environment you were exposed
to, caregiver, developmental milestones, relationship with other children, complications at birth,
exposure to violence as you grow, etc.).
B. Educational History (Primary, middle school, high school, college, honors received and its
impact to your life, drop out problems, suspension problem, learning problem etc.)
C. Family History (birth order, current residence of family, mother’s occupation, father’s
occupation, brothers or sisters and their work, closeness of the family, extended family, problems
encountered within the family, punishments or abuse in the family etc.)
D. Social History (friends, best friends, close friends, groups you belong in the university, your
groups outside the school, your social relationships with other people, your view about your
relationship with others etc.). Please use here the attachments styles test you have taken.
E. Physical History (physical health concerns, history of physical illness in the family, influences or
impact of physical health to current self, medications etc.). Please click the link below to have an

F. Psychological History (counseling, assessment, treatment or intervention received, serious
mental health concerns such as hopelessness, suicidal attempts or thoughts, sleep problems,
appetite, emotional concerns, disturbing thoughts or behaviors, self-esteem low, etc.). Please take

G. Essential Self History (religious affiliation, spiritual life, prayer life, religious beliefs, religious
practices, rituals, traditions, feelings about own religion, spirituality etc. Satisfaction with one's
gender; feeling supported in one's gender, Cultural Identity: Satisfaction with one's cultural
identity; responsibility for one's wellness through self-care and safety habits that are preventive in
nature). Also, please determine your spiritual gifts by taking the gifts test below and reflect on it
in this part of the self-analysis. The gifts test is a free, brief and through multiple choice survey.

H. Creative Self History (The combination of attributes that each of us forms to make a unique
place among others in our social interactions and to positively interpret our world. Being mentally
active, open-minded; Being aware of or in touch with one's feelings; Beliefs about your
competence, confidence, and mastery (i.e., "I can"); Being satisfied with one's work; Being able
to laugh at one's own mistakes and the unexpected things that happen).

I. Coping Self History (The combination of elements that regulate our responses to life events and
provide a means for transcending their negative affect. Activities done in one’s free time:
satisfaction with one’s leisure activities, General perception of one's own self-management or
self-regulation; Accepting who and what one is, positive qualities along with imperfections;
Understanding that perfection or being loved by everyone are impossible goals, and having the
