    AFFIDAVIT OF BARANGAY

    1. That I, ROMEO O. CELLACAY , am the incumbent Punong Barangay of Tabajan, Guindulman


    Bohol.
    2. That we knew that REGINE RANCE is Filipino of legal age, single and presently residing at
    Tabajan, Guindulman Bohol.
    3. that we knew she have 2 children is under my care since their father died last June 2020
    4. that we know she is a solo parent keeping and rearing the 2 children and she took the
    responsibilities being a single parent just to support them for their education , needs and for
    our daily consumptions.
    5. that I am executing this affidavit to attest to the truth of the foregoing facts and for whatever
    legal purpose and Intent it may serve.

    IN WITNESS WHEREOF, I Have hereto affixed my signature this 3rd day of January 2019 in the
    barangay Tabajan, Guindulman Bohol, Philippines
