    Dear Sir,
    Re: Purchase order for procurement of Welcome Kits
    We refer to the Request for Proposal (RFP) no. HO/STN & SF/ 114 dated 07.05.2022 and addendum in
    respect of Printing, Personalization of Welcome Kits. We also invite n;ference to your acceptance of the
    rates of various components of Welcome Kit vide letter dated 11-08-2022 .

    Bank has now decided to place order for Printing, Personalization & Supply of Welcome Kits to our
    branches / offices and associated Logistics / Administration, with following major conditions:

    1. Welco~e Kit would adhere to exact technical specifications a~ specified in the RFP
    2 . M/s MCT Cards & Technologies Ltd is required to provide Welcome Kit/ Service as per the detailed
    "Scope of Work" and other "Terms and Conditions" of the RFP dated 07-05-2022.
    3 . Bank will not pay any one time setup or any other charges towards the setup for preparing and
    supplying Welcome Kits. The integration with Bank's system will be done in co-ordination with
    Bank's team. .
    4. M/s MCT Cards & Technologies Ltd. will have to sign Master Service Agreement, Service Level
    Agreement and Non-Disclosure Agreement with Bank as per the format approved by Bank's Legal
    Department.

    • The actual number of welcome kit to be supplied would be as per the details provided by
    the Bank from time to time and completion of the earlier work order to the satisfaction of
    Bank and as per the timeliness specified. It may be noted that Bank, at its sole discretion,
    may not place further order(s) if the timeliness are not adhered to or in case of
    unsatisfactory support by the company.
    • · M/s MCT Cards & Technologies Ltd has provided performance Bank guarantee of
    Rs.90,26,280.00 (Rupees Ninety Lakhs Twenty Six Thousand Two Hundred Eighty
    only) vide BG no 00071IG220000045 of Indian Bank as per terms of RFP.
    • M/s MCT Cards & Technologies Ltd is required to ensure the security and confidentiality
    of Bank's data at all times during the printing & supply of the Welcome kit and afterwards.
    All necessary precautions are to be taken to ensure that all confidential information of
    Bank's customers is kept secret and used for any other purpose with prior clean written
    permission from the Bank. .,
    • As per terms of RFP, you are allotted 30 % of the entire work at L- 1 rates under the said
    arrangement. The list of zones allotted to you and rates are as under
