Chapter 1

INTRODUCTION

1.1 Background of the Study

Wellness is traditionally defined as a holistic approach to health,

encompassing physical, mental, emotional, and social well-being. This concept has

evolved from a focus solely on physical health to a broader understanding that

acknowledges the multifaceted nature of well-being. As modern life introduces

greater stress and challenges, interest in wellness practices has surged. The World

Health Organization (WHO) reported that non-communicable diseases (NCDs), such

as heart disease and diabetes, accounted for about 74% of global deaths in 2021, many

linked to lifestyle factors (WHO, 2022). This alarming statistic emphasizes the

importance of preventive health strategies and wellness.

Mental health issues have also risen dramatically, particularly exacerbated by

the COVID-19 pandemic. According to a CDC survey, symptoms of anxiety and

depression significantly increased, especially among young adults and essential

workers (CDC, 2020). The Global Wellness Institute noted that the wellness

economy, valued at $4.9 trillion in 2019, faced a decline due to the pandemic but

rebounded to $5.6 trillion in 2022, reflecting an increased global focus on health and

wellness (Global Wellness Institute, 2023).

Research supports the benefits of wellness in improving quality of life and

reducing chronic disease burden. Studies indicate that wellness interventions,

including regular physical activity, mindfulness, and balanced nutrition, can

significantly lower chronic disease risks and enhance mental health (Hertel et al.,

2020). For instance, mindfulness-based interventions have been shown to reduce

anxiety and depression symptoms (Goyal et al., 2020).

In the Philippines, the healthcare system faces dual challenges from

communicable and non-communicable diseases. Despite improvements in healthcare

infrastructure, access remains limited, especially in rural areas. According to the

Department of Health, cardiovascular diseases, diabetes, and cancer are leading

causes of death, driven by poor lifestyle choices (Department of Health, 2021). The

National Nutrition Council reported a 37.2% prevalence of overweight and obesity

among Filipino adults in 2019, worsening public health concerns (National Nutrition

Council, 2020).

As the Philippine continues to struggle with communicable diseases, such as

tuberculosis (TB), dengue, and HIV/AIDS. TB remains to be the country’s most

persistent health challenge, with the World Health Organization (WHO) estimating

554,000 new TB cases in 2019, making the Philippines one of the highest TB –

burdened countries globally (WHO, 2020)

Mental health in the Philippines has historically been overlooked. However,

the passage of the Mental Health Act in 2018 marked a significant step toward

integrating mental health care into public health systems. The COVID-19 pandemic

worsened mental health issues, with nearly 18% of Filipino adults reporting moderate

to severe depression symptoms in 2021 (Garcia et al., 2021). Despite this, there

remains a critical shortage of mental health professionals, with only 2.2 mental health

workers per 100,000 population (WHO, 2021).

Bacolod City reflects broader trends and challenges in the Philippines.

Although interest in health and wellness is growing, significant barriers remain.

Limited access to wellness services disproportionately affects lower-income

communities, where wellness offerings are often seen as luxuries (Medina & Garcia,

2020). Additionally, Bacolod faces rising rates of lifestyle-related NCDs, attributed to

unhealthy diets and sedentary lifestyles, with the city recording some of the highest

rates of hypertension and obesity in the region (Department of Health Region VI,

2021).

Mental health challenges in Bacolod mirror national trends, compounded by

stigma and inadequate access to care. Despite the Mental Health Act, the city lacks

sufficient trained professionals and facilities (Ramos & Panganiban, 2020). The

COVID-19 pandemic led to increased anxiety and depression among residents,

particularly among younger individuals and frontline workers, with 25% reporting

moderate to severe anxiety symptoms (Garcia & Lopez, 2021). Despite the rising

demand for mental health services, the city’s capacity to address these needs remains

inadequate.

Accordingly, environmental health issues further complicate wellness in

Bacolod. Deteriorating air quality, poor water quality, and inadequate waste

management significantly impact physical and mental health, particularly in lower-

income neighborhoods (Department of Health Region VI, 2021). Moreover, a lack of

public awareness about preventive health measures exacerbates the situation, with

many residents unaware of the benefits of a healthy lifestyle.

Wellness is important because it integrates various dimensions of health such

as physical, mental, emotional, and social that all contribute to our overall well -

being and quality of life. But despite the growing demand for wellness services in the

Philippines, it still faces challenges like limited accessibility, growth, and long-term

sustainability. Addressing these challenges through regulation, increased accessibility,

public education and support from the government. This study will not just answer the

growing need of wellness centers but will also give professionals in the wellness

industry a chance to practice and better the community through holistic approach.

To address these challenges, there is a need for improved accessibility and

sustainability of wellness services in Bacolod. This study aims to develop community-

based wellness programs tailored to local needs. Improve access to health services,

particularly for lower-income residents and create a community center where

residents can engage in health-promoting activities and education.

In conclusion, while the wellness movement is gaining traction in Bacolod City,

addressing existing barriers is crucial for fostering a healthier community. By

prioritizing holistic health approaches and increasing accessibility to wellness

services, we can improve overall well-being and quality of life for residents.

1.2 Statement of the Problem

The purpose of this study is to entitled “A Wellness and Integrative Health

Complex” is to present an analysis and to determine a functional, efficient and

pleasing architectural design for a wellness center that will cater physical and mental

health facilities to the needs of the citizens in Talisay City.

Specifically, the following questions are to be considered:

1. Site Analysis

a. What is the status of the proposed site of the project to qualify for the land

use and zoning regulations as to the construction of the proposed project?

b. How easy is it for staff and visitors to access the site and its facilities?

c. How does the current peace and order situation affect the perceived safety

and well-being individuals in the community?

d. How does the surrounding surround environment contribute to the overall

overall sense of wellness and quality of life in the area?

e. How reliable and accessible are the basic utilities such as power, water,

and communication systems, and how does it influence the community’s

wellness.

2. Site Development

a. To establish Biophilic Design in a Wellness and Integrative Health

Complex.

1.3 Theoretical Framework

This study focuses on two theories, Biophilic and Empathy in Design, that

drive the design of a Wellness and Integrative Health Complex.

“Biophilic” - described as an innate human desire to connect with nature

(Kellert & Calabrese, 2015). Biophilic Design, as promoted by Kellert et al. (2011)

and Beatley (2010) globally, is a sustainable design approach aimed at re-establishing

the bond between people and the natural world. Regarding health and well-being,

Ryan et al. (2014) confirmed the importance of biophilic design, citing research in

fields such as neuroscience and endocrinology that demonstrates the positive

psychophysiological and cognitive effects of incorporating nature into design.

The therapeutic benefits of nature on both physical and mental health have

been extensively explored. The connections between human well-being and nature are

multi-faceted, encompassing both direct and indirect effects. Beyond fulfilling

fundamental needs like food and resources, nature has been shown to heal and

alleviate various, positioning it as a vital component of health maintenance (Dushkova

et al., 2020). Historically, the sanctuary of Asklepios at Epidaurus, situated on a hill

with fresh air and verdant views, served as an ancient Greek rehabilitation center,

demonstrating nature’s early role in physical healing (Maas J. et al., 2009).

In the same year, Kellert et al. outlined four core principles of biophilic

design: (1) the necessity of continuous engagement with nature; (2) adaptation to the

natural world; (3) fostering emotional connections to specific environments; and (4)

encouraging positive interactions between people and nature, promoting a broader

sense of responsibility toward natural communities. Additionally, they emphasized

the importance of interconnected and integrated architectural solutions (Keller et al.,

2011).

William Browning introduced 14 biophilic design patterns in 2014 (Browning,

Ryan, & Clancy, 2014), while Stephen Kellett and Elizabeth Calabrese proposed 24

biophilic design strategies in 2015 (Keller, 2015). A comparative analysis of these

frameworks shows that biophilic design evolves in three ways: by using real natural

elements, abstracting and extracting natural characteristics, and transforming the

relationship between humans and nature. Browning's approach categorizes the first

two methods based on their simplicity and introduces the concepts of "mystery" and

"adventure" to enhance the design.

“Empathy in Design” – Empathy in design, in contrast, encompasses a broader

practice that emphasizes incorporating empathy into the design process by focusing

on the user’s experience and identifying their specific problems. This approach seeks

solutions that resonate with the emotional and psychological needs of users. It

prioritizes understanding users' emotions over merely addressing functional needs,

offering services that align with their feelings, values, and experiences (Mediastika,

2016).

An empathy-driven design process in healthcare environments enhances the

holistic experience for users by adopting an experiential approach. This process helps

connect quality architectural design with patient-centered care (Carmel-Gilfilen &

Portillo, 2016). Implementing inclusive design in healthcare facilities is essential for

ensuring effective, efficient, and safe care delivery for both patients and staff.

Additionally, it promotes equity in healthcare by addressing the diverse needs of the

population and minimizing barriers to access (McClung & Linton, 2024).

The designing and planning of the proposed Wellness and Integrative Health

Complex will adapt the Biophilic concept of modern architecture. The theory that the

researcher has chosen would like to visualize the relationship between the building

parts and its forms and functions working.

1.4 Conceptual Framework


A Wellness and Integrative Health Complex in Talisay City

The diagram outlines the comprehensive study process, initially establishing

the rationale for the study and delineating the interrelations between variables.

Subsequently, from the input of the main idea of this research and that is to design

and plan a wellness and integrative health complex, it progresses through the

architectural design phase, encompassing building planning, aesthetic considerations,

utility integration, and material selection, all crucial aspects necessitating thorough

planning and research. Concurrently, principles promoting safety, sanitation,

specialized medical services, advanced technology, research, education, and training,

the anticipated outputs of this research, are incorporated. These principles underpin

the overarching goal of achieving a safe, technologically advanced, and educationally

conducive healthcare environment. The ultimate outcome is to meticulously design

and plan a wellness and integrative health complex tailored to meet the specific

healthcare needs of Bacolod City while taking into consideration its culture, tradition

and spatial context. This will result in a facility that is able to provide high-quality

medical care and improve the community's general well-being.

1.5 Significance of the Study

The study will be focusing on the proposed project entitled “A Wellness and

Integrative Health Complex in Talisay City”. The project will be most significant for

it will help individuals in the field of architecture in the desire to put up a wellness

and integrative health complex that will cater to people who want to better their health

through good health practices. Furthermore, this study will be significant to:

The People of Negros Occidental

This study will be significant for it will help provide a well-planned

architectural design of a Wellness and Integrative Health Complex. This shall become

a reference for whatever related provincial proposals and projects especially in the

promotion of agriculture and in developing a new form of relaxation in the province.

The City of Talisay

This study will be considerable for the City of Talisay for it will be a good

reference for the progression of tourism in the city and will help people better their

health.

The People of Talisay

This study will be the orientation and reference for those people who are

interested in wellness and health related programs in Talisay City. This study will be

the reference to help understand the people of Talisay City to comprehend the

importance of health and well-being.

Architecture Students

The findings of this study will provide information related to the topic “ A

Wellness and Integrative Health Complex” which could help architecture students

who plan to take similar studies or proposals.

To the Researcher

This study will give the researcher the opportunity to put all that he has

learned in this course into reality. This further widens his knowledge of certain facts

and features related to the study, which eventually developed his awareness to the

needs of the society. This study will also make the researcher realize that this final

course requirement is not just a gateway to pass the course, but for the researcher to

grow subsequently.

Future Researchers

This study helps future researchers by providing a comprehensive reference on

the state of wellness and integrative health in Talisay City. It offers insights into the

specific challenges and opportunities within the local context, shedding light on

factors that influence wellness, such as socio-economic conditions, accessibility of

health services, and cultural attitudes towards holistic health practices. By raising

awareness of integrative health approaches, the study encourages future exploration of

innovative strategies to improve community well-being.

1.6 Scope and Limitations

The study aims to develop a facility that provides accessible quality healthcare

for the residents of Bacolod City and Talisay City. It will assess the need for a

Wellness and Integrative Health Complex by identifying gaps and challenges in

creating an inclusive space for the community. The research will also seek to

understand public perceptions regarding a health center focused on wellness.

This study will not address other medical facilities beyond wellness centers

and health complexes. While broader topics could be explored, time constraints limit

the scope of this research.

1.7 Definition of Terms

The Definition of Terms section is essential for clarifying the key concepts

and terminology used throughout the study. This ensures that readers and future

researchers have a clear understanding of how these terms are applied within the

context of the research. By defining each term precisely, this section eliminates

potential ambiguity and fosters a consistent interpretation of the study's findings and

discussions. Below, the key terms relevant to this study on wellness and integrative

health are defined to provide clarity and enhance comprehension.

Biophilic

relating to, or characterized by biophilia; relating to, showing, or being the

human tendency to interact or be closely associated with other forms of life in nature.

Operationally, biophilic is the design approach that the study revolves around.

Chronic Diseases

A disease or condition that usually lasts for 3 months or longer and may get

worse over time. Operationally, Chronic Diseases include cancer, heart disease,

stroke, diabetes, and arthritis.

Complex

A group of buildings that are designed to work together, often sharing

aesthetic elements or functions. These can include housing developments, office

parks, or educational facilities (Ching, F.D.K, 2014). Operationally, a facility that

offers a range of services and amenities, such as fitness centers, medical care,

therapeutic treatments, and holistic wellness programs, all designed to promote

overall health and well-being.

Community

Community refers to a group of individuals or populations sharing common

interests, values, and goals, often within a specific geographical area. (Smith, M. K.,

2016). Operationally. It refers to a practical and functional definition that focuses on

how a group of people interacts and collaborates within a specific context.

Emotional Well-Being

Emotional well-being is when people manage emotions well and have a sense

of meaning, purpose, and supportive relationships (CDC, n.d.). Operationally,

Emotional Well-Being refers to an individual's ability to effectively manage and

express emotions, maintain a positive outlook, and cope with stress or challenges in

daily life.

Fitness

It is the condition of being physically strong and healthy (Cambridge

Dictionary, n.d.). Operationally, Fitness is typically defined as through structured

programs, training regimens, or physical activities designed to enhance an individual's

physical capabilities and overall health.

Health

The condition of the body and the degree to which it is free from illness, or the

state of being well (Merriam-Webster, n.d.). Operationally, health can be evaluated

through medical examinations, diagnostic tests, or wellness assessments, and it is

maintained or improved through preventive care, treatments, healthy behaviors, and

participation in health and wellness programs.

Holistic Approach

A support that looks at the whole person, not just their mental health needs.

The support should also consider their physical, emotional, social and spiritual

wellbeing (NSW Health, n.d.). Operationally, refers to addressing all aspects of a

person's well-being, including physical, mental, emotional, social, and spiritual health.

Integrative Health

A concept that aims for well-coordinated care among different providers and

institutions by bringing conventional and complementary approaches together to care

for the whole person (National Center for Complementary and Integrative Health,

n.d.). Operationally, it involves coordinated care between healthcare providers from

different disciplines to create personalized treatment plans that address the full

spectrum of health—physical, mental, and emotional.

Lifestyle-related diseases

prolonged exposure to three modifiable lifestyle behaviors -- smoking,

unhealthy diet, and physical inactivity (United Nations, n.d.). Operationally, lifestyle-

related diseases are health conditions primarily caused or exacerbated by unhealthy

behaviors such as poor diet, lack of physical activity, smoking, or excessive alcohol

consumption.

Mental Health

It is a state of mental well-being that enables people to cope with the stresses

of life, realize their abilities, learn well and work well, and contribute to their

community (WHO, n.d.). Operationally, refers to an individual's psychological and

emotional well-being, including the ability to cope with stress, work productively,

maintain relationships, and make decisions.

Physical Health

It is the condition of your body, taking into consideration everything from the

absence of disease to fitness level. (EUPATI, n.d.). Operationally, physical health

refers to the functioning of the body’s systems and the absence of illness or disease. It

is often measured through medical assessments, physical exams, vital signs, fitness

tests, and laboratory diagnostics.

Social Well-Being

The term refers to building and maintaining healthy relationships and having

meaningful, authentic interactions with others. Operationally, social well-being refers

to an individual's ability to form meaningful relationships, engage in social

interactions, and feel a sense of belonging within their community.

Talisay City

It is a fourth-class city in the province of Negros Occidental, Philippines

(Talisay City Government, n.d.). Operationally, it is the city where the project is

located.

Wellness

It is the act of practicing healthy habits on a daily basis to attain better

physical and mental health outcomes, so that instead of just surviving, a person

thrives (Pfizer, n.d.). Operationally, wellness is a proactive approach to maintaining

and improving overall health and well-being, encompassing physical, mental,

emotional, and social aspects of life.

Wellness Center

These are facilities that house health care professionals such as life coaches,

nutritionists or alternative medicine specialists (Hammons, n.d.). Operationally,

wellness center refers to a facility that offers a range of services aimed at improving

individuals’ physical, mental, and emotional well-being. These centers often provide

fitness programs, nutritional counseling, mental health services, spa treatments, and

holistic therapies.

METHODOLOGY

This chapter will discuss the different details in methodology that the

researcher will employ in the following: research design, population and sampling

technique, research instrument, its validity and reliability, data gathering procedures,

and statistical treatment of data.

2.1 Research Design

This study will use descriptive design. This design is the most appropriate

since the study deals with different areas of architectural design such as site analysis,

site development, building function, building aesthetics, and building technology and

utilities and would need specific information as part of the input for the design.

To gather the information needed for the proposal, the researcher will conduct

a survey to determine the functionality of putting up a wellness and integrative health

complex in Talisay City. Initial observation, site visitation and documentation were

done to get the background of the site and were considered valuable inputs in creating

the questionnaire.

2.2 Research Locale

The study will be conducted in Bacolod City, located in Negros Occidental,

Philippines. Bacolod is an urbanized area with a population exceeding 500,000

people, known for its vibrant healthcare sector and growing wellness industry

(Philippine Statistics Authority, 2021). The city serves as a central hub for healthcare

services, catering to residents from both the city and surrounding towns like Talisay

(Department of Health, 2019).

The research will focus on wellness centers in Bacolod City, which offer a

range of integrative health services, such as mental health counseling, physical

therapy, and nutritional consultations (Health Wellness Council, 2022). These centers

were chosen because they represent a cross-section of wellness services available in

the city, making them ideal for studying the impact of integrative health on residents.

Accessibility is straightforward due to the city’s developed transportation

infrastructure, though residents in more remote areas may face challenges in reaching

the centers (City Planning and Development Office, 2020).

2.3 Sources of Data

The data for this study will be collected from both primary and secondary

sources. Primary data will be obtained directly from first-hand sources, such as

professionals or experts in the field of health and wellness. Secondary data, on the

other hand, will come from second-hand sources, including journals, articles, and

other written materials that the researcher has not personally experienced. These data

will provide the essential information needed for the study.

Primary data will be collected through direct interactions with individuals who

have firsthand knowledge of the subject. This will include:

Existing Site Conditions

Assessing the conditions of the proposed site is crucial when creating a viable

building design for a project. This evaluation determines whether the location is

appropriate for the planned facility. Important factors such as accessibility, existing

infrastructure, environmental considerations, and local regulations must be considered

to ensure that the site can accommodate the design and operational needs of the

project. Conducting a comprehensive site analysis helps guarantee the project's

feasibility and long-term sustainability.

Interviews

Conducted with healthcare professionals, wellness practitioners, and staff

working in wellness centers.

Surveys and Questionnaires

Distributed to patients and clients of wellness centers in Bacolod City and Talisay

City to gather their perceptions and experiences regarding the services offered.

Focused Group Discussions

Organized with residents from both cities to explore their views on the need for a

Wellness and Integrative Health Complex.

Secondary Data

Secondary data will be sourced from existing literature and published materials.

These will include:

Academic Journals

Articles and studies related to wellness centers, healthcare accessibility and

integrative health practices.

Government Reports

Health statistics, wellness industry trends, and regional healthcare infrastructure

reports from local government units and the Department of Health.

Texts related to health and wellness design, patient-centered care and healthcare

inclusivity.

Online Databases

Peer – reviewed articles and case studies from databases like Google Scholar and

the like.

2.4 Sampling Technique

The researcher will employ simple random sampling and will have a minimum

sample of two hundred (200) participants to answer the survey questionnaire, half of

the participants from Talisay City and the other half from Bacolod City, in order to

achieve the approximate number of samples. The minimum sample mentioned above

was based on the population size of Talisay City 2025 and Bacolod City 2025

projected population. From the 2015 PSA actual census, Talisay City has 106,850

residents and Bacolod City has 641,653 residents currently (Philippine Statistics

Authority, 2022).

2.5 Inclusion Criteria

The criteria set by the researcher to meet the requirements and gather the

relevant data. Inclusion criteria include factors such as age, gender, health status,

location and occupation.

a. Age range: Participants must be between 18 and 65 years of age.

b. Location: Participants must be residents of Bacolod City or Talisay

City.

c. Health Status: Participants must have visited a wellness center at least

once in the past six months.

d. Occupation: Participants must be either healthcare professionals,

wellness practitioners, staff working in wellness centers or patients.

2.6 Data Gathering Instrument

The study will employ survey questionnaires to collect critical data on

elements that cannot be directly observed, such as the opinions and preferences of the

respondents. Information will be gathered using standardized methods, ensuring that

all participants are asked the same questions in a consistent manner. This ensures

uniformity in data collection across the respondent pool.

The questionnaire will begin with a section on the respondents' profile, where

basic information such as name (optional), location, gender, age, civil status, and

occupation will be collected. Following this, the survey will be divided into three

sections: part two will cover planning, part three will focus on building aesthetics, and

part four will assess respondents' satisfaction. The questionnaire will be designed to

be easy to answer, ensuring that the questions are both user-friendly and provide

valuable insights for the development of the proposed project.

2.7 Data Gathering Procedure

Upon approval of the survey questionnaire, the researcher will conduct a

survey to gather all the information on the opinion of the people in the areas of

Bacolod City and Talisay City regarding the establishment and the design of the

proposed Wellness and Integrative Health Complex. Furthermore, observations,

interviews, library research and other documents will be collected through photos and

recording of data to ascertain facts in architecture. Subsequently, the researcher will

then collect all the data gathered, and the results will be statically treated.

2.8 Data Analyses

Data Analysis is the process of systematically applying statistical and/or

logical techniques to describe and illustrate, condense and recap, and evaluate data.

Formula to calculate the appropriate sample size from the total population. To

2.9 Ethical Considerations

This study will follow ethical guidelines to protect the integrity and welfare of

all participants involved in the research. The ethical considerations to be implemented

are as follows:

Informed Consent

Participants will receive clear and thorough information regarding the

study's purpose, procedures, risks, and benefits before consenting to

participate. Informed consent will be documented in writing, and participants

will retain the right to withdraw from the study at any time without any

repercussions.
