Box 17768 Baltimore, MD 21235 Re: Notice of Improper Use of Social Security Number Dear Sir or Madam: This letter confirms the conversation I had with Marissa Taylor at the Fraud Hotline on April 05, 2012. Recently, there has been improper use of my social security number. For identification purposes, my social security number is 123-45-6789 and my date of birth is April 03, 1988. The improper use of my social security number occurred when someone used the number to file a fraudulent tax return. The fraud was discovered when I reviewed my tax records from the IRS. I am requesting that your office conduct an investigation into this improper use. Please provide written confirmation that an investigation is under way.
