WHAT IS THE PURPOSE OF A PTW SYSTEM?
• Define the scope of the work
• ID Hazards and assess the risks
• Establish Control Measures (eliminate/ mitigate).
• Links work to other permits/ certificates
• Authorized by responsible persons
• Communicate the above to all persons involved
• Ensure all hazards have been identified; appropriate control
measures have been taken and communicated

ROLES & RESPONSIBILITIES
Operations Manager
HSE Manager
• Ensure the PTW procedure is up to date (custodian)
• Monitor compliance to PTW procedure
• Ensure integrated risk assessment (JHA) process
ROLES & RESPONSIBILITIES
Permit Issuer
• Full understanding of the scope of the work
• Ensure all hazards and controls are identified
• Ensure the Permit Receiver understand the terms
• Has to ensure that work site preparations are done
• Ensure that there is no conflicting activities on site
• Suspend or close the PTW when it is no longer
required
• Be present on site at all times during works
ROLES & RESPONSIBILITIES
Permit Receiver
• Complete the PTW
• Ensure that all attachments are made as required
• Prepare the work area as defined in the PTW
• Discuss the permit fully with Permit Issuer
• Understand the limitations and restrictions of the PTW
• Inform the Permit Issuer and PTW Office of any changes
• STOP the work if it is unsafe to continue
• Carry out housekeeping on a daily basis
• Communicate all of the above to his work team
• Carry out the gas test with the help of authorized gas tester
• All permits should be submitted for at least 1 day before for
review and approval
