A 12 month old infant was brought to OPD with history of recurrent


vomiting
Gastroesophageal Reflux Disease

GERD

 It is increased exposure of esophagus to gastric and duodenal


secretions.
 It’s a normal process in infants and becomes a problem when its
recurrent or causes pathological symptoms eg of esophagitis or
respiratory problems or nutritional compromise
 85 percent of premature infant have GERD
PATHOPGYSIOLOGY
 Main factors that contribute to GERD are:

 Liquid Diet
 Supine Position
 Short esophagus
 Small non-compliant stomach
 Frequent large volume feeding
 Immature LES
 Transient LES Relaxation
CLINICAL FEATURES

 Most common symptoms are of esophageal pathology like esophagitis


such as:
 Irrataibility
 Arching
 Choking
 Gagging
CLINICAL FEATURE

 Extra Esophageal symptoms can be of respiratory system


like:
 In Infancy Obstructive apnea, stridor or Lower airway
disease
 In older children symptoms are related to asthma,
laryngitis, sinusitis
COMPLICATIONS
 Failure to thrive
 Esophagitis with hematemesis
 Anemia
 Esophageal Stricture
 Aspiration Pneumoniae
 Chronic cough
 Wheezing
 Dysphagia
 Dental erosions
 Sandifer syndrome
DIAGNOSIS

 Barium Swallow
 Esophageal pH monitoring
 Upper GIT series
 Esophageal Impedance Monitoring
 Endoscopy
MANAGMENT
 Conservative Treatment:

 Erect Posture and start on solid food


 Frequent small feedings thickened with rice cereal (2-3
TSF/oz of milk)
 Burp more frequently
 Stick to a regular feeding schedule
 Keep the baby away from smoke
 Avoid chocolates and beverages
Medical Treatment

 Omeprazole
 Metoclopramide
 Ranitidine
Surgical Treatment

 Indicated in:
 Persistent vomiting with failure to thrive
 Esophagitis or esophageal sphincter
 Apneic spells
 Chronic pulmonary disease

 Surgical option is Fundoplication with


Gastrostomy
