Ethical Issue:
Confidentiality
z
Code of Ethics for Physician Assistant

 Autonomy

 Beneficence

 Nonmaleficence

 Justice
z
Autonomy

 Autonomy is the belief that the

patient has the right to make
choices in concern to their
treatment and advanced
directive or living will

 As physicians' assistants it is
our goal to honor and respect
the patient's decisions and
guide them with the correct
medical advice
z
Beneficence

 Beneficence is the best interest

of the patients, as a PA it is our
medical duty to do no harm to
our patents and give them the
best care, they need for the
situation
z
Nonmaleficence

 Nonmaleficence is to do no
harm or put an extra burden on
the patient that could increase a
health risk
z
Justice

 Justice is the same care for all

patients that are in the same
circumstance

 Another way to look as justice is

anti-discrimination in the
healthcare setting.

 As physician assistants our goal

is to treat everyone and help
everyone no matter who they
are.
z
Confidentiality

 Protected under Healthcare Insurance Portability and

Accountability Act (HIPAA)

 Under this act medical professionals cannot speak about

patients without their consent no matter the circumstance

 Having confidentiality gives the patients trust in the healthcare

staff especially physician assistants and allows for them to seek
treatment more often and help dig up underlying issues.
z
Scenario 1

 Imagine as a physician assistant working in orthopedics you

have a 15-year-old male patient that plays football, seeking
treatment for a high ankle sprain. As the evaluation begins, the
father starts talking about him being the star freshman
quarterback for the rival high school.
z
Scenario 2

 One night I was visiting with my grandparents and they asked

me about a certain resident and how they were doing since they
heard over their police scanner that an ambulance was being
sent over to our facility. Not thinking I explained the situation and
told the story of what happened. Not thinking anything about it I
returned to work and was pulled into my supervisor's office to be
asked questions if I broke HIPPA, I explained yes, I was fired
and had to write a formal apology to the family.

Review

 The PA code of ethics states

 “Any communication about a

patient conducted in a manner that
violates confidentiality is unethical.
Because written, electronic, and
verbal information may be
intercepted or overheard, the PA
should always be aware of anyone
who might be monitoring
communication about a patient.”

 Applies to all situations on how patient

information can be thrown around from a
casual conversation
z
Solutions

 Storage and transmission of patient

information that reduces the chances
of data becoming available to the
public.

 Consensus decision making outside in

public life

 “PAs should advocate for policies and

procedures that secure the
confidentiality of patient information”

 ("Guidelines for Ethical Conduct for

the PA Profession", 2013).
