There are several options for reporting different types of incidents to Westminster College departments. Please review the following options and select the one most appropriate for your needs.

None of these reporting forms are 911 or Emergency Services. If you require emergency assistance, contact Westminster College Public Safety at (724) 946-7777 or dial 911.

NOTE: Campus Security Authorities (CSAs) are required to report crimes via the CSA Report form which can be found here: https://www.westminster.edu/campus/services/csa-incident-report.cfm

 

This form should be used to report conduct that does not necessarily violate College Policy, but involves unwelcome conduct based on a person's race, color, creed, religion, national origin, sex, sexual orientation, gender identity, ability, age, marital status, or any other protected class status.
This form should be used to report discrimination based on race, color, creed, religion, national origin, sex, sexual orientation, gender identity, ability, age, marital status, or any other protected class status and/or prohibited conduct which includes harassment, sex-based harassment, sexual assault, dating violence, domestic violence, stalking, intimidation, non-consensual sexual contact, non-consensual sexual intercourse, sexual exploitation, or retaliation.
This form should be used to report incidents of sexual harassment, sexual assault, dating violence, domestic violence, or stalking as defined under Title IX.
This report form should be used to report a student or student organization for violating College policies found in the Student Handbook.
