Unfortunately, reports regarding the specific drop location and circumstances are conflicting - some say the item dropped during high-tier Nightmare dungeon farming, while others state it dropped in Champion's Demise on World Tier 4.

I saw a build for Necro that used this as it's best in slot. I'll just say, if your build requires one of the rarest and best equips in the game to even function, its a bad build.
Considering it gives +4 to all skills and 20% dr, its bis for everyone.
