HOPE FAMILY SERVICES (TRP) BRIGHTON PARK

Provides individual and group support to victims of Domestic Violence. Services include counseling support for both the victims and for people who have caused harm. Hope Family Services serves adults and children/youth in a family.

UI HEALTH DAVIS HEALTH AND WELLNESS CENTER

This community health center is located at Nathan S. Davis Elementary School and offers services to both Davis Elementary School students and members of the community. Offers primary care, dental screenings, vision screenings, and behavioral health services.

DEVICES 4 THE DISABLED

Provides free, gently used, durable medical and mobility equipment, including wheelchairs, lifts, and hospital beds, to individuals and families in need.

Provides a range of primary, specialty, and emergency medical care. Services are provided regardless of ability to pay.

ACCESS COMMUNITY HEALTH

Provides full primary health care services, OB/GYN, and low-cost breast and cervical screening. All patients are welcome, regardless of insurance status. Patients without the ability to pay will not be denied services.​

Our mental health services section offers resources and information on mental health providers and services, including therapists, counselors, and support groups. SAINT ANTHONY HOSPITAL: COMMUNITY WELLNESS PROGRAM

Provides free, bilingual (Spanish and English) individual and group mental health therapy for uninsured and underinsured adults and adolescents. They additionally provide therapeutic family support services, including individual and group parenting support and couples and family therapy.

SINAI HEALTH SYSTEM

Provides bilingual (Spanish and English) inpatient and outpatient mental health services for adults, including mental health assessments, case management, psychiatry services, and individual, family, and group therapy. Additionally offers bilingual (Spanish and English) mental health therapy and psychiatry services for children and adolescents through the Under the Rainbow program.

Offers inpatient and outpatient mental health services to children and adolescents, as well as inpatient and intensive outpatient mental health services for adults. Accepts most types of insurance and may also provide financial assistance to individuals who are eligible.

MIDWEST ASIAN HEALTH ASSOCIATION

Provides a range of primary, specialty, and emergency medical care.

Provides one-on-one counseling for individuals who have experienced domestic violence. Services are free.

CENTRO SANAR AT PORT MINISTRIES

Provides free, bilingual (Spanish and English) individual and couples mental health therapy and case management to adults. Additionally offers group support, including group support for individuals who are experiencing grief and loss and a Spanish Dialectical Behavior Therapy (DBT) group that teaches skills for managing difficult emotions.

Offers school-based counseling to address students’ mental health needs at six schools in Brighton Park (Brighton Park Elementary School, Burroughs Elementary School, Davis Elementary School, Shields Elementary School, Shields Middle School, and Thomas Kelly College Preparatory).

Our education section provides resources for students of all ages, including information on schools, scholarships, financial aid, and career development. Whether you're just starting your educational journey, looking to further your career, or seeking opportunities for continuing education, our guide can help you find the resources you need to succeed.

SGA YOUTH AND FAMILY SERVICES

Provides mentoring and case management services to high school juniors, seniors, and college students at Kelly College Prep (FAFSA applications, explore scholarships, and assist with job readiness skills).

METROPOLITAN FAMILY SERVICES – PARENTS AS PARTNERS HOME VISITING PROGRAM

Provides free, weekly home visiting services to mothers and fathers between the ages of 13 and 24, whose family is expecting or has a child age birth to three years old.

Provides center-based early childhood education for families with children up to age 5. The center is located at 5338 S. Loomis Blvd. Services are free.

Provides home-based early childhood education and developmental screenings for families with children up to age 5. Center-based early childhood education and parent resources are also offered at the new Early Learning Center. Services are free.

CENTER FOR INDEPENDENCE THROUGH CONDUCTIVE EDUCATION

Provides group-based intensive motor training programs for children and adolescents with Cerebral Palsy and other physical disabilities. Additionally offers a range of support programs including sibling support groups, parent education nights, home-visit training programs, educational advocacy, medical advocacy, and comprehensive case management services. Programming is also available for young adults ages 18-25, with a focus on socialization skills, activities of daily life, community integration, and vocational training. Services are provided to individuals who are insured and uninsured, regardless of ability to pay. The Center has a Brighton Park location at 2701 W. 36th Place.

BRIGHTON PARK NEIGHBORHOOD COUNCIL – COMMUNITY SCHOOLS

Provides structured educational and enrichment activities for children and teens, including homework and studying help, through its after-school programs. Additionally offers English as a Second Language (ESL) and high school equivalency classes and testing for adults. Services are free.

AFTER SCHOOL MATTERS

Provides after-school and summer program opportunities to Chicago high school teens. Chicago Teens participate in high-quality, hands-on, project-based apprenticeship and internship programs in various content areas, including the arts, communications and leadership, sports, and STEM. Eligible teens receive stipends for completion of program requirements.

Youth Violence Prevention Resources:

Our Youth Violence Prevention Services section offers resources and information on violence prevention programs and services, including after-school programs, mentorship programs, and community outreach. Whether you're a young person looking for support and guidance, or a concerned parent, educator, or community member seeking to prevent youth violence, our guide can help you find the resources you need to make a difference.

NEW LIFE CENTERS OF CHICAGOLAND

Provides mentoring and group programming to youth (ages 12-24) who are at risk of becoming involved in the criminal justice system. Group programming includes trauma informed peace circles, adventure play therapy, cognitive behavioral therapy, and sports based programming. They also offer a pop-up food pantry at New Life Church at 4155 S. Rockwell St. on Tuesdays from 4:30-6:30pm.

Financial Support Resources

Our Financial Support services section provides resources and information on financial assistance programs, including government programs, non-profit organizations, and community resources. Whether you're struggling with debt, facing financial hardship, or seeking information on how to manage your finances, our guide can help you find the resources you need to get back on track and achieve financial stability.

SINAI COMMUNITY INSTITUTE

Provides assistance completing applications for the Women, Infants and Children (WIC) supplemental nutrition assistance program. Is also distributing baby formula, baby food, diapers, and wipes to support families impacted by the COVID-19 pandemic.

MIDWEST ASIAN HEALTH ASSOCIATION

Provides assistance to help people learn about and apply for their health insurance options.

GREATER CHICAGO FOOD DEPOSITORY

Provides assistance with applying for Medicaid insurance benefits and the Supplemental Nutrition Assistance Program (SNAP). Can also help individuals determine whether they may be eligible for Women, Infants and Children (WIC) benefits, the Low Income Home Energy Assistance Program (LIHEAP), and tax credits (EITC).

LADDER UP

Offers free tax preparation services, financial coaching, and assistance completing the Free Application for Federal Student Aid (FAFSA) college financial aid application. Can also assist taxpayers in applying for and renewing an Individual Taxpayer Identification Number (ITIN). Offers a Legal Tax Clinic for low-income individuals and families who have a controversy with the IRS or Illinois Department of Revenue.

BRIGHTON PARK NEIGHBORHOOD COUNCIL – PUBLIC BENEFITS ENROLLMENT

Offers support applying for health insurance benefits and the Supplemental Nutrition Assistance Program (SNAP). Additionally assists eligible households in reconnecting or paying for gas, propane, electricity, and/or water through the Low Income Home Energy Assistance Program (LIHEAP).

Provides comprehensive case management services and financial coaching to help families at risk of homelessness find stable housing and increase financial stability.

Our Job Training and Employment Resources section offers resources and information on job training programs, employment services, and career development resources. Whether you're a recent graduate looking to start your career, an experienced professional seeking to further your skills, or someone looking to re-enter the workforce, our guide can help you find the resources you need to achieve your career goals. From resume-building workshops to job placement services, our guide can connect you with the tools and resources you need to succeed in the job market.

WORKING TOGETHER CHICAGO

A program of Center for Independence Through Conductive Education, Working Together Chicago provides employment support for young adults with disabilities. Services include employment development and placement, short-term job coaching, and long-term mentoring.

Provides one-on-one job counseling for individuals who are searching for employment or interested in learning new skills that will help them find a job. Additionally offers training programs including English as a Second Language (ESL) classes, customer service training, and programs that prepare individuals for careers in banking and insurance sales. Offers Empresarias del Futuro, an educational training program in Spanish for women interested in beginning or expanding their small business.
