Heart Attack (Ischemic Heart Disease) is a medical disorder that happens when the coronary arteries become blocked reducing blood flow to the heart muscles.
The blood supply to the part of the heart can not give sufficient oxygen and nutrients to the muscles of the heart.
The cells in that affected area of the heart died off and that area of the heart is not able to function.
This is called a heart attack.
Heart Attack (Ischemic Heart Disease) can result in sudden death.
It needs just four to six minutes after cardiac arrest before a person develops brain death and then complete death.
The survival rate before reaching a hospital is less than 1-2%.
A heart attack (myocardial infarction) happens when blood supply to the heart muscle stops and the heart muscle dies.
A sudden cardiac arrest (SCA) happens when the heart suddenly stops pumping due to an electrical disorder in the heart.
Heart Attack (Ischemic Heart Disease) happens when
1. There is an obstruction of an artery in the heart by a blood clot, piece of fat or air bubble traveling in the blood from another part of the body.
This results in a lack of supply of oxygen and nutrients to the heart muscle.
Consequently the heart cells died resulting in loss of function of the part of the heart.
2. The narrowing of the blood vessel because of fat deposit in the walls of the artery supplying blood to the heart.
This decreases the blood supply of oxygen and nutrients to the heart muscles cells and decreases its function.
The causative diseases are:
1. High Blood pressure-Hypertension narrows the arteries of the heart and provides blockage.
2. High cholesterol-Cholesterol deposit causes the walls of the arteries to be thicker, making them narrow and reducing the blood flow.
3. Diabetes mellitus
4. Obesity
5. Smoking and alcohol
The main symptom of IHD is a crushing heaviness or pressure in the chest which may spread to the left jaw, neck, shoulder and down the inside of the left arm and hand.
2. Breathing difficulty is normally linked with the chest pain
3. Cold sweats
Diagnosis of ischemic heart disease is normally confirmed by:
ECG (electrocardiogram) reveals characteristic features of depression in Q wave, ST segment and raised RS segment.
Angiogram (cardiac catheterization)
Immediate treatment is required
Give nitroglycerin under the tongue if nitroglycerin is available
1. Anticoagulants such as warfarin, aspirin
2. Vasodilators like isorbide
3. Any abnormal rhythm of the heart must be treated with medicines
Surgical treatments include:
1. Balloon angioplasty
2. Stent
3. Bypass heart surgery

Ischemic Heart Disease is the third commonest cause of death in Singapore in 2014 having fallen from second place in 2009 to cancer and pneumonia.
I had several patients who died of heart attacks most of them peacefully.
Some of them had a successful coronary artery bypass operation done on them but later succumbed to other illness such as pneumonia.
Treatment has been quite successful in reducing the number of heart attack especially with the use of blood thinners such as aspirin, plavix and warfarin that prevent formation of blood clots in the coronary arteries.

Since 2013, I have tried to improve my spelling and writing.
As I tried to bring you the latest information about a condition or illness by reading the latest journals both online and offline, I find that I am learning more and improving on my own medical knowledge in diagnosis and treatment for my patients.
Just by writing all these simple guides I find that I have learned a lot from your reviews (good or bad), criticism and advice.
I am sorry for the repetitions in these simple guides as the second chapters onwards have new information as compared to my first chapter taken from my blog.
I also find repetition definitely help me and maybe some readers to remember the facts in the books more easily.
I apologize if these repetitions are irritating to some readers.

Heart disease is the commonest cause of death in both men and women in the USA.
Cancer and stroke are the two other diseases on the top three
Heart disease encompasses disorders such as coronary artery disease, heart attack, cardiac arrest, congenital heart diseases and congestive heart failure.
Heart disease is responsible for 40% of all U.S. deaths more than all forms of cancer combined.
The most frequent cause of heart disease is ischemic heart disease (IHD) which is an obstructed or narrowed coronary artery that delivers blood to the heart.
A person with both a first-degree relative (a parent or sibling) and a second-degree relative (uncle or grandparent) who had suffered from heart disease before the age of 60 is nearly 10 times more likely to have from heart disease early in life.
CT scans of mummies show that heart disease was surprisingly common in ancient Egypt suggesting that heart disease is induced by factors other than current bad habits such as smoking, fast food and inactivity.

People who stay up late may be more susceptible to heart disease even if they get eight hours' sleep.
Women who slept five hours or less a night were 39% more likely to have heart disease than women who got eight hours.
All of this is possibly due to habits or events linked with late nights or short sleep hours rather than the time factors themselves.
Heart attacks and sudden cardiac arrests are two different episodes.

People who stay alone are two times as likely to have a heart attack or sudden cardiac death as those who live with a partner or roommate
What are the causes of Ischemic Heart Disease?

The high blood sugar level forms triglycerides that can thicken the arteries and make the blood thick so that less blood is capable of flowing through them.
4. Obesity
A BMI of 23.0kg/m² and above raises the risk of getting high blood pressure, diabetes and heart attack.
5. Smoking
Tobacco smoke constrict arteries and decreases oxygen supply to the heart.
6. Drinking of Alcohol
Alcohol can induce liver and kidney damage and raise the blood pressure and involve diabetes.
7. Stress
Repeated stress if poorly controlled can result in high blood pressure.
Negative emotions and depression are danger factors for heart attack and stroke.
On the other hand happier people are less prone to develop heart disease.
8. Poor Dental Health
People with poor oral health may be more likely to have atherosclerosis (clogged arteries) because the bacteria in the gums and teeth can pass into the bloodstream and affect blood clotting.
9. A woman’s resting pulse rate was a good predictor of her risk of heart attack.
Women with high heart rates at or above 76 beats per minute were more prone to have a heart attack than women with reduced resting pulse rates (62 beats per minute or less).
Six and a half million Americans complain of angina (severe cardiac pain).
About 2,500 Americans have deaths from heart disease daily equaling one death every 34 seconds.
Age is the most significant danger factor of forming heart disease followed by gender, family history together with ethnic background, smoking, and obesity, lack of exercise, high blood pressure, diabetes and high blood cholesterol.
About 40% of people having a heart attack pass away before they reach the hospital.
Heart medicines may have serious interactions with herbal supplements such as ginkgo biloba, St. John’s wort and garlic.
Menopausal women are more prone to getting heart disease because their bodies form less estrogen.
Two-thirds of deaths from heart attacks in women happen in those who have had no history of chest pain.
Kawasaki disease and acute rheumatic fever are the two main causes of secondary heart disease in children in the United States.
Hypertension or when the pressure in the arteries is higher than normal raises the danger of coronary artery disease (atherosclerosis).
High blood pressure normally has no symptoms and not only injures the heart but the kidney and the brain as well.
Laughing relaxes and widens blood vessels which can help protect the heart.
A person is more prone to have an ischemic heart attack on a Monday morning rather than on any other morning of the week.
A large dinner with high fat and carbohydrate calories can narrow blood vessels, raising the risk of blood clotting.
