Veterinarian's Assistant chatimg
Customer: My dog is losing hair in patches on his side and stomach. The patches are irregularly shaped and the fur scrapes off with what looks like thick dandruff attached to it. He’s not itching the spots and they aren’t red.
Veterinarian's Assistant: I'll do all I can to help. What is your dog's name and age?
Customer: Charlie, 10 years old
Veterinarian's Assistant: Is there anything else the Veterinarian should be aware of about Charlie?
Customer: It’s really late. Is there really a veterinarian up right now who wants to answer my questions? I can chat back tomorrow.

The spots aren’t discolored and just feel like skin. He’s overweight and has chronic ear infections.
Hello. This is Dr Mike. I have been practicing for 19 years and I will be happy to help you today with all of your questions. First, I would like to start with a few questions to help me better understand your case.
Can you send some pictures of these areas for me to review ? I have seen this condition many times.
An underlying Medical conditions?
Customer
Here are a few.
Customer
No medical conditions. Just chronic ear infections.

HOw long have we had ear infections? What have you been using to treat them?

HOw long has the hair started to fall out like this and not grow back? Are we putting on weight? Slowing down? Harder time getting up and around?

Customer
The hair loss we noticed a few weeks ago. We gave him an oatmeal bath thinking maybe he had gotten into something that was irritating his skin, but it just keeps happening.
Customer
He’s had ear infections on and off his whole life. We’ve tried everything and they still happen. Currently I am using a 50/50 solution of rubbing alcohol and apple cider vinegar to try and keep them clean, but he still smells like it’s infected. It goes away after a couple days of the drops.
I am going to send you a protocol that is very inexpensive and successful for the ears now.
The ear canal is in the shape of an "L", this allows for the ear canal to become very moist and warm, then the opportunistic yeast and bacteria that normally live in the ear canal can start to grow to higher levels. The key is we need to flush the ears twice a day to start with, This will remove the debris and the chlorhexidine will also kill the yeast and the bacteria in the ear canal. Then once the ears get healthier we can decrease on an as needed benefit. You may need to do from time to time to keep the ears healthy. The miconazole and the ketoconazole will also help with the yeast too. This product can be purchased over the counter at pet stores or online like on Amazon
Customer
We’ve tried the over the counter ear cleaning kits for prevention. We’ve been told by vets to change up how food, which we’ve done numerous times. He’s had antibiotics. We had a whole refining prescribed with about five bottles of stuff to use. We were told he needed to be allergy tested for $1200 and that he could be allergic to grass. We’ve tried Zymox.

Does the hair also seem more brittle, just kind of falls out almost and then does not grow back?

Customer
It kind of feels like straw.

18 years ago I found this CHlorhexidine ear flush, it has decreased my repeat ear infection over 90%. The key is to keep the ear canal clean of debris and kill the yeast and bacteria. Most ear flushes I find just remove the debris, not kill the yeast and bacteria. Just wanted to share this with you to help. I practiced most of my career in the lake regions of Minnesota, we always have ear infections up there.

Have we put on any weight over the last year?

Customer
I see it comes in spray or a big jug. Do I use the spray?
Customer
No, he’s actually lost a little weight over the past nine months. We had him on this current dog food, then I switched him to a RAW diet for about four months, when he lost some weight, then we put him back on this dog food, but that was several big bags ago, so maybe three months. He’s maintained the lost weight since going back on it.

The ear flush will come in like a 12 or 16 ounce bottle. It has an end which is very nice for administering in the ear canal. Many different companies make it. It can have different names. Chlorhexidine is the main component to have in it. If you can find one with miconozole or ketoconazole, even better.

When we start to see this hair just falling out, more dandruff, not growing back in and getting less healthy. Many times we are concerned that hormonal (endocrine) problems are occurring. Hypothyroid is a top differential when this occurs. Low thyroid will make the hair go into a resting phase, it will fall out much more easily, is weaker, less healthy. When this occurs it also grows back in much slower, so the bald spots stay there. Then the skin is less healthy also, so we see more dandruff and flaking of the skin too. I would recommend to have a thyroid test done to assess for this. Low thyroid will also make the ear infections worse also. Is there any yeasty or musty odor to the body now or near the sores?

Customer
No. Just the ears. The spots themselves don’t have any dandruff or flaking skin, just the edges. The skin is smooth and clean.

It can start like this, the areas many times like on the sides are friction points, where we get up and down, lay on, this rubs that hair off more hear than elsewhere at the beginning. You can also use a medicates shampoo that also has chlorhexidine and miconozole in it (one brand is called Malaseb), there is a spray called Micochlor that has the same ingredients in it too that you can spray on these. However my biggest concern on these cases is why the hair is falling out and not growing back in. That is why I mentioned testing the thryoid level. If it is low, you can start on a thyroid supplement, once the thyroid level comes up to more normal levels, then is maintained, more normal and healthy hair will start to grow back in.

Customer
Okay. Will do. Thank you so much for your help! We just want to get some answers.

I always like to look to find the underlying cause, not just treat the clinical signs. This is why you can do as I mentioned to work on the skin and ears topically. However being able to find the potential underlying cause to make us feel better overall will be so much more healthy for us. Any other questions I can answer for you now?

Customer
Nope. I’ll call a local vet and get him tested right away. Thank you!’
