Q. What to do in urine retention in a pt of birth asphyxia with HIE. Alt hot and cold compresses are not effective. As far as I know, manual compression is harmful. The baby passes urine only when compressed.

Please check to rule out an outlet obstruction. Secondly, there may be cord involvement in case of severe HIE and you may have a neurogenic bladder. An initial USG with post void residue will give a clue. Thereafter, consider CIC, indwelling catheter for a week and reassess.

Q. A child has a history of either Asthma or Recurrent wheeze-associated LRI. In case of Fever in both.......

If the child has a fever, then NSAIDs would be required. One may use paracetamol or ibuprofen.

Expert Opinion:
Rule out brucellosis, and enteric fever. Monitor ESR. If it rises also rule out autoimmune disorders.

Q. Can we give prophylactic A.T.T., to asymptomatic, P.E.M Gr. 1, 2,5 year below children, with Mantoux -ve, with poor appetite, and normal other investigations? How do we manage these types of cases?

Expert Opinion:
There is no need to give prophylactic ATT to such children except if there is sputum-positive family contact. This is more of a feeding problem. We have to look at Birth weight, child was LBW or not, feeding history, and serial weight record if available to see when the child faulte red in growth, rule out Urinary infection and then individualize the management - Nutritional rehabilitation.
