Milwaukee Field Hockey Club Regional Team

Dear Athlete, Milwaukee Regional- the regional team is for players that are very serious about their commitment to field hockey but do not have the time necessary to commit to our Elite program. This program is our attempt to provide consistent high level practice and tournament opportunities. The regional teams are required to train at least once per week. These players participate in local competition only. How does the program work? We will have two teams. Regional Blue is based in Mequon and Regional White is based out of Wauwatosa. To be on the team, its a first come first serve situation. We will only take 12 athletes per team maximum. We are also exploring having a third team based out of Merton. Players that do sign up in time for team blue or team white will be referred to this other team. These teams will practice with each other weekly and play against other teams (MKE regional, MKE travel and Chicago) regularly. We are looking to provide a tournament opportunity every month. Team 1- Regional Blue: Team practices on Sunday afternoons from 4-5.30pm at Wilson School Gym. Practices start November 24th. Team 2- Regional White: Team practices on Friday evenings from 7-8.30pm at McKinley School Gym. Practices start November 22nd. Tournaments- approximately once per month the regional team will compete against local competition. Local competition also includes teams from Chicago, other MKE regional teams, MKE travel teams and North Shore Field Hockey Club. Practice costs- We are trying to keep costs to a minimum for this program. We will have two methods in which an athlete can pay for practices. 1. Turn up and pay $10 at the door. 2. Commit to all Nov-April practices for an average cost of $8. (total cost $112)

Tournament Fees: Local Tournament cost depends on length of play but usually $25. Local Tournament vs. local club teams: one per month- Usually $25 each tournament that lasts over 4 hours. Tournament fees are paid on or before each tournament date. *Northwestern Tournament May/June 2014- (Chicago)- $150 (Tournament Fee due April) This is a select tournament and is not required. *Firecracker St. Louis July 2014- Again, select tournament and is not required. *Out of the regional teams we will select several athletes to represent MKE in the Northwestern and Firecracker tournaments. A few additional practices will be required. This is not mandatory for all of our athletes. Other Costs: Uniform: Practice pinnie/uniform $38.99 Year round MKE Membership fee: $75 USA Field Hockey Membership: $50 Checklist *Please remember that this is a first come, first serve sign up. 3. Purchase your Practice pinnie/uniform. 4. Choose your method of paying practice fees. Select this option to pay all November to April practices for a discounted practice rate of $8. Total cost of $112. Or Please bring $10 to each practice. We have a pay to play policy, athletes must pay practice fee before each practice. 5. Sign up for the team online here. Athlete must complete both sign up form (selecting team preference) and the online waiver. 6. Complete athlete letter of intent and bring it to the rst practice.

Milwaukee Field Hockey Regional Athlete Letter of Intent Dear Milwaukee Field Hockey Club, Date:_____________________ I______________________ accept the invitation to be part of the Regional hockey team. I understand that I am committing to attend the majority of scheduled practices and tournaments. I agree to pay the required fees and I will not be eligible for a refund if I can no longer commit to the program. I will communicate any conflicts throughout the season to my coaches ahead of time. I will represent the Milwaukee Field Hockey Club with respect for myself, my coaches, my teammates, my parents and my competition. I agree to make safety of paramount importance by wearing appropriate clothing and field hockey equipment, removing all jewelry and following coaches instructions. I understand that my responsibility as a player is to contribute my best effort and competitive drive to my team while maintaining quality sportsmanship both on and off the field.
