While processing your application the Federal Department of Human Services (DHS) advised us that the information you provided below did not match the information they have on file: Your Name: Steven Poon Your DHS Customer Reference Number: 207232710S DHS must be able to identify you and determine if you were eligible for Family Tax Benefit A or B during the 2012-2013 financial year, and that you received the relevant payment. This is an eligibility criterion for the 2013-2014 Family Energy Rebate. Unfortunately, this means we can not continue to process your application, and must close it. However, you can submit a new application, preferably using the online application form, to hopefully resolve this issue of a mis-match of information with DHS, to continue the process of checking your eligibility for the rebate. The fastest path to the online application form is this address: http:// www.energy.nsw.gov.au/customers/rebates/. Here are some suggestions to help you resolve this issue before submitting a new application before the submission deadline of 5:00pm, Friday 13 June 2014. 1. Contact the DHS Customer Service Number 13-61-50 to discuss the Customer Reference Number issue. Once this is resolved, lodge a new application with your updated information. 2. A common error is submitting a Customer Access Number rather than a Customer Reference Number in the rebate application. The Customer Reference Number is the number on your DHS correspondence and concessions cards. Discuss this with DHS if you are still uncertain as to what is your current Customer Reference Number.
