CLINICAL EXEMPLAR
During my preceptorship I had many excellent learning experiences. One of these experience
included a patient in acute respiratory distress that was deteriorating. I will describe this situation using
in this clinical exemplar. The purpose of a clinical exemplar, according to Black (1997), is to allow a
nurse to highlight critical thinking skills is a situation they experienced and showcase all of what
nursing entails.
My patient was a three year old girl with a history of asthma and had been admitted for
respiratory distress related to pneumonia. Her assessment showed tachypneia, diminished breath
sounds, subcostal and intercostal retractions, and congested cough. Her O2 saturations where in the
mid-to-low 90s and decreasing despite being on oxygen. She was also reporting pain in her abdomen
which we attributed to the increased work of breathing. She was also just visibly uncomfortable. She
was withdrawn and tearful at time.
We gave her toys to attempt to cheer her up and continued to assess. She received many
respiratory treatments and was eventually started on vapotherm. Despite this, her work of breathing did
not improve very much and she continued to be in distress. The physician ordered subcutaneous
epinephrine, IV solumedrol, and contacted All Children's PICU in order to have her transferred.
I learned a lot from observing and participating in this patient's case. It showed me what
assessment pieces to look for in a patient with deteriorating respiratory distress, and when to look into
sending them to a higher acuity unit. I also learned a great deal about the transport process as I
witnessed the transport nurses prepare the patient for the helicoptor flight to All Children's.
