Standards for Blood Banks and Transfusion Services, 27th edition
Effective Date: May 1, 2011
New!
The 27th edition of Standards for Blood Banks and Transfusion Services details the latest standards of practice in blood banking and transfusion medicine. As in the past, a quality management system is the framework of the standards, with each of the 10 chapter headings representing one of the 10 Quality System Essentials. Significant changes: A new (previously interim) standard requires the use of FDA-approved or FDA-deemed-equivalent bacteria detection methods. new requirements ensure that users follow manufacturers instructions with certain apheresis equipment when said instructions are more stringent than standards. Standards for the preparation and issue of tissues and derivatives have been separated from those for blood and components, for clarity. Requirements for new product Cryoprecipitated AHF after thawing have been added to Reference Standard 5.1.8A, Requirements for Storage, Transport and Expiration. The phrase transfusion transmitted has been replaced with the more inclusive term transmissible for clarity.

The fifth edition of Standards for Cellular Therapy Product Services details the latest standards of practice for accredited cellular therapy facilities. Significant changes: new standard requires that informed consent documents be reviewed and approved by the medical director. new standard requires a plan for the eventual implementation of ISBT 128 labeling for cellular therapy products. Several revisions to infectious disease testing requirements including changes to requirements for autologous donor testing. Several standards requiring inspection and verification, particularly at the time of issue or distribution, have been revised to allow for the use of one individual and an electronic equivalent instead of requiring that two individuals verify labeling information.
(AABB, 2011, soft cover, 126 pages, ISBN 978-1-56395-327-9)
The 10th edition of Standards for Relationship Testing Laboratories (RT Standards) features new and revised requirements in areas such as personnel responsibilities, first-time accreditation, the use of autosomal markers, testing of backup systems and corrective action. The various fields represented by these six different Standards are symbolic of AABBs continued effort to adapt to changing clinical and laboratory environments and to address the expanding needs of its membership. In 1957, AABB published the first edition of Standards for Blood Banks and Transfusion Services. The Standards were instituted in 1958, and AABBs Accreditation Program came into existence. More than 50 years and 25 editions later, AABB continues to be a leader in standard setting and accreditation. AABBs Standards and Accreditation Programs have evolved over time but are still supported by the work of volunteers and continue to strive for improvement in the quality and safety of blood collection, processing, storage and transfusion activities, as well as in the areas of cellular therapy, perioperative activities, molecular testing, immunohematology and relationship testing.

This timely book examines the policy and clinical decisions being made today in the name of blood safety, often resulting in radical changes in transfusion medicine practice. Although decision making in transfusion medicine is based on a wider range of inputs than solely the best research evidence, the orientation of this book remains evidence-based. In this capacity, the book is intended to serve one of the recurring principles enunciated by the FDAthat decision making must be transparent. The book discusses both the evidence supporting and the hotly debated policy alternatives proposed for avoiding the risks of allogeneic blood transfusion.
With patient safety and optimal outcomes as the goal, blood management and utilization concepts reach across diverse areas of transfusion medicine, from physician education and auditing to surgical techniques and topical hemostatics, to include all aspects of the transfusion decision-making process. Representing a major revision of the Guidelines for Blood utilization Review (AABB, 2001), this Guideline assists facilities in developing strategies that provide for the appropriate use of blood and the use of alternatives that help to reduce or avoid the need for allogeneic blood transfusion. A multidisciplinary approach is advocated that includes the use of blood component guidelines, pharmaceutical preparations to minimize blood loss, blood conservation methods and blood utilization review practices. Some highlights include: The transfusion review process, including thresholds and assessments. nonsurgical as well as preoperative, intraoperative and postoperative strategies. The blood utilization review process, including the development of facility-specific guidelines and quality improvement. Auditing of blood utilization, including criteria for a wide range of components and situations.
(AABB, 2011, soft cover, 52 pages, ISBN 978-1-56395-326-2)
Topics: The contrast between precautionary principle and evidence-based medicine. Causes of, and strategies to reduce, transfusion-related mortality. Emerging transmissible infections. Pathogen reduction. MSM donor deferrals. Patient blood management. Meta-analysis and equipoise in transfusion medicine. The book includes a bonus CD-ROM containing three fundamental chapters from Evidence-Based Practice of Transfusion Medicine, which together provide guidance for independently assessing the quality of published studies such as those cited in the books chapters.

Pathogen Inactivation:
AABB brings you the first comprehensive reference on transfusion-related lung injury (TRALI), collecting all the latest research in one timely volume. This accessible tool provides both an overview and a detailed picture of the relevant clinical, laboratory, pathophysiologic and donor-related issues posed by TRALI. Intended to provide transfusion medicine professionals, transfusing clinicians and other staff with an up-to-date reference guide, each chapter has been written by a recognized authority in the field, summarizing existing knowledge as well as highlighting areas of ongoing investigation. Topics: Differential diagnosis, incidence, pathophysiology and treatment. Concerns in the ICu. Donor management and laboratory evaluation. HLA and granulocyte antibody testing. Experience in the uK. Risk reduction.
(AABB Press, 2008, soft cover, 196 pages, ISBN 978-1-56395-267-8)
In this imperfect world, pathogen inactivation has been sought as a way to achieve a zero-risk blood supply. Although no group of techniques can be relied upon as a true panacea, pathogen inactivation is a bold leap in a positive direction. With the assumption of remarkably low risk, albeit at a higher cost, these techniques hold promise for addressing not only the known safety concerns, including bacterial contamination, but also the next new virus for which we are unprepared. This comprehensive and timely volume presents the basic concepts, reviews the research performed to date and provides practical implementation advice from an international panel of early adopters. Topics addressed include: The rationale for pathogen inactivation. The challenges posed by some targets of inactivation. Assessing the toxic effects. Methods and considerations for red cells, platelets and plasma. Practical experience from several European sites. Regulations and implementation from both north American and European perspectives. This text will be of interest to those who are already using pathogen inactivation techniques (to compare and refine their operations), practitioners who are considering pathogen inactivation for their facilities in the near future (to identify aspects for consideration), and those who just want to be more knowledgeable about it (to be ready when asked to step up).
(AABB Press, 2010, soft cover, 294 pages, ISBN 978-1-56395-309-5)
This Guideline addresses the clinical and laboratory features of transfusion-related acute lung injury (TRALI), as well as its management. TRALI is a life-threatening complication of transfusion that is clinically indistinguishable from the acute respiratory distress syndrome (ARDS), but which has a more favorable prognosis. nevertheless, in the developed nations, it is the second or third most common cause of death from transfusion.
(AABB, 2003, soft cover, 5 pages, ISBN 1-56395-202-5)
With the societal expectation of a zero risk product, the high cost society has been willing to pay for meager improvements in risk, and increasing concerns about emerging infectious agents, pathogen inactivation may not be as far off as we think.

(AABB, 2009, ISBN 978-1-56395-303-3)
While infectious transfusion complications are often the focus of patients, physicians, blood transfusion professionals and regulatory agencies, nonviral and immunologically based complications are the most prevalent risks for a transfusion recipient. The diagnosis and evaluation of any transfusion reaction is a matter of clinical judgment by a physician trained in the adverse effects of transfusion. negative testing results should not be interpreted as definitely excluding a transfusion reaction if there is a strong clinical suspicion that the patients symptoms are related to blood transfusion. The laboratory evaluation of suspected transfusion reactions is critical not only to the health and well-being of the patient but also to the process of improvement through corrective and preventive actions.

Many hospitals and transfusion services are now using pneumatic tube delivery systems as a means of delivering blood products faster and more efficiently. To ensure product safety and the maintenance of quality, system validation is key. The goal of this Guideline is to provide direction when considering pneumatic tube technology for blood component distribution. For facilities currently using this technology, this serves as a comparison between legacy methods and those addressed by the current validation. For institutions using legacy pneumatic tube systems, this document can help direct procedures needed to maintain long-term quality assurance goals.

Guidelines for Implementing an Electronic Crossmatch
Developed for the Scientific Section Coordinating Committee by Suzanne H. Butch, MA, MT(ASCP)SBB, CQA(ASQ)
This Guideline explains the practical aspects of switching from serologic techniques to electronic procedures for crossmatching. After a description of the advantages of the electronic crossmatch, as well as the hurdles that remain for those wishing to make the change, the document addresses the details of process design. Included are discussions of positive patient identification, accurate data entry, specimen requirements, unit type confirmation, override functions, key aspects of training/competency assessment, computer validation, billing issues and performing a hazard analysis.
(AABB, 2003, soft cover, 34 pages, ISBN 1-56395-181-9)
This Guideline reviews the ethical and legal basis of the consent doctrine and suggests elements of the consent process that should be included in both the donor and recipient settings. It also reviews the roles of the physician directing a phlebotomy, the blood bank and transfusion service medical directors and the treating clinician. The goal of this guidance document is to help readers achieve high-quality consent processes at their institutions.

Donor Services Training Everything You Need to Know to Process Donors, 9.0
The Continuing Education Committee originally prepared this easy-to-use resource, and the Administrative Section Coordinating Committee/Scientific Section Coordinating Committee updates it to reflect changes in the AABB Standards for Blood Banks and Transfusion Services. This manual is an effective training tool for both the classroom and individual study settings. Highlights:
Outline of regulatory and accreditation requirements. Overview of blood components. Principles of donor selection. Collection of blood. Special procedures.
Anemia in prospective blood donors may indicate an underlying disease, may result in an ineffective blood product and may cause harm to the recipient. Thus, the measurement of donor hemoglobin as a means of determining donor eligibility is of paramount importance. This Guideline identifies several of the methods currently in use for determining hemoglobin levels. The advantages, disadvantages, technical aspects and quality assurance considerations of each method are described so that collection personnel can choose a particular method that ensures protection of the donor and a safe and effective product for the recipient.
