Letter of Recommendation Request Questionnaire for Mr.

Shah
Please type responses to the following questions in complete sentences. Please be as detailed and thoughtful in your responses as possible. Most importantly, Id like for you to include a few stories/incidents and include them in some of your responses. Examples: If you have had a transformation, was there a moment that you recall where this change started happening? Describe that moment and contextualize it. If you helped students outside of class, talk about why you did that and maybe write a short paragraph describing the scenario if it was memorable? I write good recommendations, but the great ones are the ones with stories, details, and I try to come up with them on my own. But you know yourself better than I know you, and you know what happens outside of the classroom better than I do, and so Id like to use that to bring more of you into the recommendation. The more thought you put into this, the better the letter that I can write.

Read over the questions below, and before you start answering them, think about something that is not covered in these questions, but you wish were covered. Then say it here. Why are you asking me to write your letter of recommendation? How did you grow as a student by taking our class? Has there been an evolution or transformation that has happened to you in your math learning/your feelings towards math? Not necessarily in this class, but in your entire math career? What are you most proud of in your work in our class? What are you least proud of in your work in our class? Was there a particular unit or topic that we covered that was memorable to you and if so, why was it memorable? Was there a particular moment in class that you remember distinctly? Tell me about that moment. What do you consider to be your best student and personal qualities and how did you demonstrate these in our class? What student and personal qualities do you need and/or want to improve on? Why? What specific steps will you take to accomplish this? Was your academic performance in our class affected by any personal obstacles (illness, stress, etc.)? If so, talk about how you managed to overcome these difficulties. What do you think you want to study in college, and why? How do you see yourself 5 years from now? 10 years from now? 20 years from now? Add anything else that you think would help me tell a college about you and would differentiate you as a student amongst your peers what made you unique in our class?
