1. On April 25, 2010 at about 10:00oclockin the morning a shooting
incident transpired at So. Hanawa, Brgy. Matangcong, Sigma, Capiz
wherein victim one Nestor Dela Torre Y Urdas, 46 years old, married,
laborer, and a resident of So. Hanawa, Brgy. Matangcong, Sigma,
Capiz, was shot by suspect Harry Baradero YLegaspi, 47 years old,
married, farmer, and a resident of the same place. As a result
thereof, the victim sustained multiple gunshot wounds in the
different parts of the body and was rushed to Mambusao General
Hospital Mamusao, Capiz for medical treatment but was pronounced
dead on arrival (DOA) by the attending physician thereat. The
suspect was arrested at his house during hot-pursuit operation
conducted, but the weapon used in the commission of the crime was
not recovered and he is now under the custody of this station for
investigation and proper disposition. The cadaver of the victim was
moved to Regional Crime Laboratory Office, Camp Martin Delgado,
Fort San Pedro, Iloilo City for autopsy examination.
2. The motive of incident is undetermined and the case still under
investigation.

Authority for Investigation:
a. Verbal orders of PSI JUAN DELA CRUZ, Chief of Investigation dated Nov. 7, 2010 to
investigate a reported robbery incident at AC Construction, San Joaquin, Pasig City.b.
Complaint of Johny Doe for robbery dated Nov. 5, 2010.II.
Initial Findings: Initial investigation disclosed that robbers entered the ACConstruction
store owned by a certain Johny Doe by destroying the back window of one of the toilets.
According to the owner, 200 bags of cement; 10 wheel barrow, and P25,000.00 cash
were missing. He named two (2) former employees; namely Hilario Doel and Pablo
Sena as the culprits who were

2. ICOW the above cited subject and reference, please be informed that on or about10:00
pm of November 3 2012 along V.V. Soliven Avenue, Cainta Rizal a stabbing incident transpired
wherein victimidentified as one
ROEL JOHN POSADAS y SANTOS, 22 yearsold, single, Student, a native of Pampanga and
presently residing at No. 10. Aquino St., Taytay, Rizal, was stabbed by the suspect identified as one
JAKE REYES y CRUZ, aka JR, 22 years old, single, laborer, a native of Northern Samar and
presently residing at No. 11 M. H. Del PilarSt., Cainta,Rizal.
3. Initial investigation conducted showed that on cited TDPO, while the victim waswalking
along VV. Soliven Avenue, Cainta Rizal way back to their house. Suddenly, he noticedJake Cruz
Reyes following him. When he reached the corner of the street, JAKE REYES y CRUZ blocked
the way of ROEL JOHN POSADAS y SANTOS and said GAGO KA!SINULOT MO
SYOTA KO!. After his words, he got his knife to his pocket and stabbed Roel
John Posadas three times that caused him fell to the ground. Jake Cruz Reyes immediatelyescaped
the place after the incident but he immediately caught by the concern citizens. Theresidents of the
place immediately helped the victim and sent to the nearest hospital that causedhim to survive from
the death.
4. Suspect was turned over to the station investigation division together with theconfiscated
evidence for proper action and disposition
