    REPUBLIC OF THE PHILIPPINES)


    CITY OF BAGUIO ) S.S

    I, JESSIE F. JAMES, of legal age, Filipino, single and a


    resident of #34 km.7, Loakan road, Baguio City, Philippines,
    under oath, state:

    1. That I am the brother of Mona Lisa D. James, who


    intends to pursue her education by attending college of
    law at the University of the Cordilleras located at Gov.
    Pack road, Baguio City for the academic yea 2017-2018;

    2. That I am hereby sponsoring and consenting my sister


    to study in the said school;

    3. That I, the affiant, am employed full-time as manager,


    with sufficient income and assets to pay for the
    applicants expenses during her entire stay in that
    school, as well as to responsibly cover any unforeseen
    expenses the applicant may incur while studying in the
    Philippines;

    4. That there is no criminal case pending in court against


    me, nor have been charged of subversion, rebellion in
    any court in the Philippines;

    5. That I am executing this affidavit to attest the truth of


    the foregoing facts and for whatever legal purpose it
    may serve.

    Affiants further says none.

    IN WITNESS WHEREOF, I have hereunto set my hand
