AFFIDAVIT OF DELAYED
REGISTRATION OF BIRTH

I, JESUSA S. GENTILIZO, of legal age, Filipino, married and


a resident of Tandayag, Amlan, Negros Oriental, after having been
duly sworn to in accordance with law, depose and state:

1. That I am the applicant for the delayed registration of birth of


my daughter JASMINE GRACE GENTILIZO;

2. That she was born in Amlan, Negros Oriental on April 27,


1981;

3. That I was delayed in registering her birth for the reason that
we were always transfering from one place to another during
the 35 years; that we have no means to pay and process for
my childs registration by reason of our poverty; and that we
never saw the need to secure for her birth certificate. In fact,
my child never had a chance to go to school.

4. That I am executing this affidavit to attest to the truth and


veracity of the foregoing facts and and in compliance with
the requirements of the Philippines Civil Registrar.

IN WITNESS WHEREOF, I have hereunto set my hand


this June 16, 2017 in Dumaguete City, Philippines.
