Dear Sir/Madam,

Re: Ms.Annette MacNamara

I am writing to refer Ms.MacNamara, a 73-year-old single woman who was admitted to our hospital
on 20/05/2009 with the diagnosis of fracture in the right wrist and laceration to the left hand that
followed an accident. She is due to be discharged on 22/05/2009.

With regard to her situation, she lives alone and is receiving an aged pensioner. She moved to a
small flat recently and feels very lonely. She has only one niece, Stella Attois, who visits her
occasionally.

During hospitalization, she has recovered well. She has received attention for her bruising in the
right shoulder, lower back and took panadol as pain killer. She has received some medication such as
karvea and normison for controlling blood pressure and insomnia. The wound is healing well.

Please note, a social worker arrange her to be visited by physiotherapist and meals on wheels
provide all her meals and also her niece will visit her at weekend for helping with housework and
shopping.

Ms.MacNamara has an appointment for removing her stitches in the hand and also follow up her
situation at 10.30 am on 31/05/09 at Out Patient Department.
I would appreciate it if you could arrange her daily home visits from Blue Nursing Service for
assistance of showering and dressing the wound. Should you require any further information
regarding Ms.MacNamara please do not hesitate to contact me.
