Read the case notes and complete the writing task which follows Name:

Age: 32 years

Marital Status: Divorced

Family: 2 daughters aged 5 and 7 years

Diagnosis: Acute appendicitis

Admission Date: 13 May 2014

Discharge Date: 17 May 2014

Social Background: Rents a ground floor apartment. Works on a casual


basis for a local bar 2 nights per week. No contact with father of children.
No friends nearby. Mother is a recovering alcoholic, gives little support.
Father passed away 10 years ago.

Nursing Management and Progress: Uncomplicated procedure.


Complaining of residual pain-refused pain relief. Refusing to eat.
Tolerates fluids. Wound healed and clear of infection. Hesitant to return
home to children (under care of distant relatives during hospitalisation)

Discharge Plan: Contact Social Worker for review. Monitor diet and
interaction with children. Rest with no heavy lifting or strenuous activity –
6 weeks

Using the information in the case notes, write a referral letter to Jane
Hardy, Social Worker, 15 bayside crescent, Macquarie Lakes,
requesting immediate assessment and management outlining relevant
information.
