PART I and PART II are worth a total of 129 points. To obtain a satisfactory nursing care plan, the student must make
a total of 77 points (60%) or higher. Students must submit Nursing Care Plan (NCP) on time. If not, 5 points a day will
be deducted.

PART 1: PATIENT ASSESSMENT (20%)

Identifies one nursing diagnosis per care plan.
The diagnosis should be the highest priority for
the patient. Ensures that the nursing diagnosis
statement is complete and written in PES,
include R/T (related to) and S/T (secondary to)
and AEB (as evidenced by).
11. Objectives/Plan Nursing Diagnosis 9
Have one short term goal. The goal is SMART
(e.g., patient will be able to ambulate 10 feet
by the end of this shift; patient will be
able to identify 5 symptoms of infection prior
to discharge)
12. Interventions: 45
Five interventions are required for each goal. It
is client specific, realistic and appropriate.
Includes rationales with interventions.
Rationales must be supported with references.

13. Evaluation 9
Evaluates whether the goal has been met. Does
not evaluate the interventions; but evaluates
the goals/outcomes. States whether the goal
