Dear MS. Taiño;

Greetings of good health!

This letter request pertains to Commission on Audit’s new circular no. 2023-2004 dated
June 14, 2023 findings on our Hazard Duty Pay. The said circular required to have a letter
request from the Local Chief Executive, since Health Workers are now already devolved
in the Local Government Units, emphasizing the nature of works, duties and
responsibilities, which are exposed to great danger, occupational risk and perils to life.

The following Public Health Workers working at the Municipal Health Office/Rural
Health Unit & Family Planning Center of the Municipality of Silago, Southern Leyte,
namely: DR. Jerome Caezar P. Castillo, designate as Municipal Health Officer, Ms.
Leodilin L. Ansale – Nurse III, Ms. Helena A. Ong, - Midwife II, Ms. Judy A. Anduyon, -
Midwife II, Ms. Elaine L. Cruzada, - Midwife I, Ms. Romalyn Palaña Monton, Midwife
I, Ms. Emma Escobella Estrera, Midwife I, Mr. Michael Mc Brian Hermosilla, - Med
Tech I, Ms. Lolita I Almosa, - RSI I, Mr. Mark M. Tomol,-Driver I (Ambulance).
Thus, I humbly request from your good office emphasizing that the Municipal Health
Unit/Silago Rural Health Unit & Family Planning Center where the PHWs (Public
Health Workers) mentioned above are exposed to various out-patients with
contagious and communicable diseases based on the New Rules on Hazard Pay Joint
Circular No. 1,2016 dated July 15,2016.
