    A. DOCUMENTS/REQUIREMENTS TO BE SUBMITTED: (3 COPIES EACH/3 folders) ( ) NEW ( ) RENEWAL


    ( ) Copies of Application Form
    ( ) Certified True Copies of License/Business Permits issued by the Local government units where the contractor or
    subcontractor operates.
    ( ) Copy of Audited Financial statements, for corporation, partnership, cooperative or union, copy of the latest
    Income Tax Return (ITR) for sole proprietorship – 5 million substantial capital.
    ( ) Certified True copy of Certificate of registration of firm with SEC for corporation, partnership or certified copy of
    registration of business name with the Department of Trade and Industry (DTI) for sole proprietorship or Certified
    copy of registration with CDA for cooperatives, or certified copy of certificate of registration with DOLE for labor
    union.
    ( ) A certified listing with proof of ownership or lease contract of facilities, tools, equipment, premises implements,
    machineries and work premises, that are actually and directly used by the contractor in the performance or completion
    of the job, work or service contracted out. In addition, the applicant shall submit a photo of the office building and
    premises where it holds office.
    ( ) A sworn disclosure that the registrant, its officers and owners or principal stockholders or any one of them, has not
    been operating or previously operating as a contractor under a different business name or entity or with pending
    cases of violations of these Rules and/or labor standards, or with a cancelled registration.
    ( ) Photo of the office and premises where the contractor holds office and Sketch of location for the business
    ( ) Visit/ocular inspection of processor before recommendation for approval. (see attached report)
    ( ) Payment of registration fee of One Hundred Thousand Pesos Only (P100, 000.00) after completion of the above
    requirements/ocular inspection.

    FOR RENEWAL (additional requirements)


    ( ) Certificate of membership and proof of payment of SSS, Phil-Health, BIR, ECC and Pag-ibig contributions for the last
    three (3) years, as well as loan amortizations; and
    ( ) Certificate of pending or no pending labor standards violation case/s with the National Labor Relations Commission
    (NLRC) and Department of Labor and Employment (DOLE).
    ( ) Copy of previous Certificate of Registration.
    ( ) Proof of submission of Contractor’s /Subcontractor’s Semi-Annual Reports.
