    AFFIDAVIT OF TWO DISINTERESTED PERSONS


    (DISCREPANCY OF NAME)

    WE, JULIUS T. FRASCO, of legal age, married, Filipino and a resident of


    Amlan, Negros Oriental and CHRISTOPHER M. ALIDO, of legal age, married,
    Filipino and a resident of _Tanjay City, Philippines, after having been duly sworn to in
    accordance with law, hereby depose and say:

    That we personally know LORENZO BETIC LIGUATON, of Escalante City,


    Negros Occidental, being acquainted for a long period of time;

    That he was a member of Home Development Mutual Fund or Pag-ibig Fund


    with ID No. 121020440039.

    That he was previously connected with Evergreen Security Services Corporation,


    1000 A. Mabi9ni cor T.M. Kalaw St., Ermita, Manila, last August 2010 to August 2013.

    That he is required by Pag-ibig Fund Office a certification but such could not be
    obtained since the office is in Manila.

    However, lately it was discovered that his name of the aforementioned employer
    was erroneously recorded as LORENZO BETIK LIGUATON instead of LORENZO
    BETIC LIGUATON.

    We are executing this Affidavit to establish the fact that the names LORENZO
    BETIK LIGUATON, and LORENZO BETIC LIGUATON refer to one and the same
    person and that the correct name should be retained on file is LORENZO BETIC
    LIGUATON.

    IN WITNESS WHEREOF, we have hereunto set our hands this 6 th day of


    September, 2019 in the City of Dumaguete City, Philippines.
