JOINT AFFIDAVIT OF ONE AND THE SAME PERSON
We, JERSON P. MORATO and HENRY L. DONAYRE both of legal age,
Filipinos, and residents of Tabango, Leyte, Philippines, after having been duly sworn to in accordance with law, do hereby depose and say that:
1. We personally know PEADELFO MONTERDE BOENAVENTURA born on
November 30, 1963 in Tabango, Leyte by ELPEDIO BUENAVENTURA SR. and GUILLERMA MONTERDE-BUENAVENTURA and our longtime neighbor and acquaintance in Tabango, Leyte;
2. That appearing on his Voter Identification Card, the name PEADELFO
MONTERDE BOENAVENTURA was entered herein and the name FILADELFO BUENAVENTURA was reflected on his Marriage Contract;
3. Further, appearing on his Marriage Contract, his father’s name was
entered as IGMEDIO BUENAVENTURA instead of ELPEDIO S. BUENAVENTURA SR.;
4. Furthermore, the said discrepancy was purely due to inadvertently, and
the said discrepancy was a mere typographical error;
5. We are not in way related to any of the foregoing person within the fourth civil degree either by consanguinity or affinity, nor do we have pecuniary interest in this matter whatsoever;
6. That hereby declares under oath that the name PEADELFO MONTERDE BOENAVENTURA and the name FILADELFO BUENAVENTURA refer to one and the same person ;
7. We are executing this affidavit to attest the veracity of the foregoing facts and for whatever other legal purpose this serves best.
