PEST CONTROL CONTRACT

We the undersigned SAM PEST MANAGEMENT SDN. BHD, a company incorporated in Malaysia (hereinafter
called “the Contractors”) and
Darul Kris Enterprise Sdn Bhd, 223, Jalan Industri Galla 6, Galla Industrial Park, 70200 Seremban, Negeri
Sembilan. (hereinafter called “the Customer”) hereby confirm that we have mutually agreed as follows:-
Service
1. 1.1 The Contractors have undertaken to make Six (6) Monthly for inspection and treatments at regular
intervals against Cockroaches, Common Ants and Rodent Control at the premises of 223, Jalan
Industri Galla 6, Galla Industrial Park, 70200 Seremban, Negeri Sembilan
1.2 The Contractor shall carry out the services as specified:
Cockroaches
- Gel or granular baiting will be applied at strategic and sensitive area to control hiding cockroaches.
- Residual spray will be carried out at external and internal perimeter of the building.
- Septic tank treatment will be practiced to eliminate cockroaches colony.
- Spot spray will be carried out for visual cockroaches.
- Survey the area for cockroaches breeding and activity area.
Common Ants
- Gel or granular baiting will be applied at strategic and sensitive area.
- Residual spray will be carried out at external perimeter, crack & crevice area
- Spot spray to the nesting area to eliminate the colony.
- Survey the ants infested area for their colony and nesting area.
Rodent
- Glueboard or rodent bait will be placed at suitable and strategic point in the building to trap rodent inside the building.
- Removing dead rodent's carcass service
* There will be an additional charges (RM 50.00) upon request for remove rodent's carcass.
- Survey the entire premise for rodent trace or mark.
Payment
2. 2.1 The Customer(s) agree/s to pay for the above services at the rate of Ringgit Malaysia: One Thousand
One Hundred Only (RM 1,100.00) for duration of the contract.
**Price Are Subject to 0% GST.
Contract
3. 3.1 The Contract is binding for a term of Six (6) Months from the date of
01 September 2019 to 29 February 2020.
3.2 This contract may terminated at the end of the mentioned term or any subsequent quarter’s by giving
Two (2) months notice in writing. Upon termination, the Customer may allow the Contractor’s customers to
enter the premises for removing all units (if applicable) from the Customer’s premises.

TERMS & CONDITIONS

1. The Customer shall immediately inform the Contractor any evidence of further activity by the pest specified
in this Contract which become known to such Customer during the service warranty period where upon the
Contractor will soonest possible carry out treatment if found necessary by their approved methods and
preparations.
2. The Contract will not apply to timbers concealed or inaccessible to visual examination.
3. Should re-infestation occur, damage may be caused but it is specifically acknowledge by the Customer, that
under this Contract, the Contractor shall not be held responsible for the damage caused or any replacement
which may be or become necessary as a result of the activity of the pest concerned but the Contractor
responsibility is limited strictly to carrying out the treatment or control of the pest concerned by the approved
methods.
4. The equipment (if applicable) which provided by the contractor will be return back to the Contractor when
the service contract ends. The Customer shall pay the contractor full compensation for any equipment which
is not returned because it is lost or stolen or any equipment which is damaged and in need of repair to put
into the same condition it was in at time of contract.
5. No variation, extension, exclusion or cancellation of this Contract shall be binding (unless terminated in
accordance with these terms) unless it is confirmed in writing and agreed by both parties.
