Head Room : Good if so then we discuss it again with the doctor today.
The patient had undergone hospitalization for a week in the hospital and today do visit
regularly by a doctor with the head of the room and the nurse on duty in the morning
shift.
(Room Nurses)
In the room the nurse, the doctor, the head of the room and nurse discuss madam adel
situation and plan further therapy.
Head Room : Doc, the patient madam adel, what should be planned for the
home, from the results of observations conducted nurses, the patient's
condition is increasingly improved and from lab results also showed
improvement. Platelets and hematocrit have also been normal. What
should go home planned for madam adel?
Doctor : Earlier also I've seen the lab results do show improved and can
be considered normal, but in my opinion should not be used to further
ensure discharged condition.
Head Room : Well, in terms of nursing care the patient was able to improve,
given nursing intervention has also been achieved and only the need for
more rest and recovery at home.
Doctor : But how will the patient's condition if it appears the fever
again? I think this patient is still a little limp and still need to undergo
hospitalization, we wait until tomorrow.
Head Room : Sorry Doc, previously essentially isitrahat patients only need
just enough to restore health, and we think it can be done at home, the
patient may not comfortable for long here. And, regarding the handling
of a fever that will eventually appear again, we are already planning
discharge planning. Discharge planning will later be given to educate
patients about the need to be considered at the house later. If later, the
fever patients appear again, will be taught to patients with warm
compresses techniques and drug therapy. because the patient did not
want to bother anyone, and he lived alone in his house.
Nurse : Yes Doc, discharge planning will later be given by me.
Doctor : Yeah if so, I expect discharge planning in the future will really be
implemented to the patient and make sure if the patient also have to
understand what to do at home
Head Room : Yes Doc, so how can the patient be discharged today?
Doctor : Yes, can. Adel patient can go home today, I will make his return
license and prescription drugs should be administered to the patient
Head Room : Miss Titi, discharge planning format which I agree and can be
performed on patients Adel. Can be prepared for discharge planning
right now.
