    AFFIDAVIT
    I, AURELIA B. OPEÑA, of legal age, Filipino, single and a resident of Purok 6,
    Barangay San Miguel, Tagum City, Davao del Norte, Philippines after having been duly
    sworn to in accordance with law, hereby depose and say:

    1. That I am the same AURELIA B. OPEÑA affiant in this affidavit and


    likewise the common law spouse of PERLITO H. OJARDA, who
    died on April 19, 2013 at Dapecol, Braulio E. Dujali, Davao del Norte;

    2. That I hereby declare that until his untimely death I have not
    contracted marriage with anybody from the time of his death and up to
    the present;

    3. That I am executing this affidavit to inform the authorities and to attest


    to the truth of the foregoing facts and for whatever legal purpose this
    may serve.

    IN WITNESS WHEREOF, I have hereunto set my hand this 24 th day of April,


    2013 at Tagum City, Philippines.

    marriage since 1987;

    for whatever legal purpose it may serve.
