    JOINT-AFFIDAVIT OF TWO (2)


    DISINTERESTED PERSONS
    WE, CRISTITO B. AUSTRAL and JEREZ P. FRANCISCO, both of legal ages, Filipinos,
    married and residents of Brgy. Tibal-og, Sto. Tomas, Davao del Norte, Philippines, after having been
    sworn to in accordance with law, do hereby depose and say that:

    1. We are the same affiants who personally and voluntarily caused the preparation and
    execution of this joint-affidavit;

    2. We personally know the GOMORY BOOC TUÑACAO, who was born on April 1, 1959 at
    Panaghiusa, Aurora, Zamboanga del Sur, Philippines;

    3. We also know that he contracted marriage with Amelia Mamac Albite on February 26,
    1982 at Digos, Davao del Sur, Philippines;

    4. We also know the fact that he is a bona fide member of Social Security System (SSS);

    5. Upon verification of their marriage contract and his SSS record, it appears that his first
    was erroneously encoded as GOMORE and GOMORIE, respectively, instead of GOMORY,
    which is his true and correct first name;

    6. It bears stressing that the erroneous entries were made due to the inadvertence of the
    aforementioned person in filling-up the said information during the registration process
    before filing the same at its respective aforementioned government offices/agencies;

    7. Being our long-time friend and neighbor, we declare the fact that the true and correct first
    name of the aforementioned person is GOMORY BOOC TUÑACAO and that the names
    GOMORE BOOC TUÑACAO, GOMORIE BOOC TUÑACAO and GOMORY BOOC
    TUÑACAO, refers to one and the same person;

    8. We are not related to the aforementioned person either by affinity or consanguinity;

    9. We are executing this joint-affidavit to attest to the veracity of the foregoing declarations,
    to apprise the authorities concerned and for whatever legal purpose this may serve best.

    IN WITNESS WHEREOF, we have hereunto affixed our signatures this October 16, 2019 at
    Sto. Tomas, Davao del Norte, Philippines.
