    AFFIDAVIT OF PARENTAL CONSENT

    I, _________________________ , 46 years old , Filipino and a resident of


    Tanglao B.E Dujali, Davao del Norte, upon oath in accordance with law, do hereby
    depose and say, THAT:

    1. I am the father of ______________________ who is 17 years old and


    presently residing with me at my above given address;

    2. Considering that he is very much interested to drive a motor vehicle despite his
    being a minor, I am inclined to permit him to drive provided that he will be
    accompanied by a professional driver in doing so;

    3. I assume the responsibility for all the circumstance that might occur as a result
    of this affidavit of consent;

    4. I am executing this affidavit to attest to the truth of the foregoing facts and for
    whatever legal purpose this may serve.
