    Declaration

    Shad_______________ have attended the “Health and Safety Training”. I do solemnly

    declare that

     I have received the safety rules and regulations for all labs of current semester and

    have signed it after reading.

     I will keep one copy of safety rules and regulations for all labs of this semester with

    me.

     I have clearly understood all safety rulesand regulations during the training.

     I have no confusion at all for ensuring the safety of my own and the people around

    me.In case at a later stage, anything is unclear, I will ask lab engineer before working

    in laboratory.

     I will act seriously in the laboratory and will report damage / accident / incident,

    major or minor, (if any) to the lab engineer immediately.

    I shall be liable to disciplinary action (immediate dismissal from laboratory, fine, pay

    damage loss, suspension for a specified period and/or even termination from university or

    whatever decided by the department) in case I am found breaching any health and safety

    rules.
