    SUBJECT: NOTICE OF BUSINESS CLOSURE

    Dear Sir:

    Our company, [Company Name], a corporation duly organized and existing in


    accordance with Philippine Laws and is principally engaged in the business of
    [consumer food manufacturing], will be closing our business due to the unrecoverable
    financial crises that we have experienced over the past few years that continue to
    deplete our resources.

    In view of this, and pursuant to Article 286 of the Labor Code, a decision to
    permanently close the business has been made, effective on [date]. Please be informed
    that our legal department is currently processing the necessary documents and
    requirements with the Securities and Exchange Commission.

    Meanwhile, in accordance to the requirements of the Labor Code, attached


    herewith is the Establishment Termination Report and the list of affected employees as
    required by your office. Separate notices have likewise been individually served to
    affected employees relative to this matter.
