Focus:

RISK INFECTION RELATED TO ASCENDING URINARY TRACT INFECTION

DATA:
A 22-year-old gravid female at 23-week gestation reported frequency and dysuria with no
other complaint. The patient presented to labor and delivery complaining of fever, chills,
and foul-smelling urine at 24-week gestation. Complained of suprapubic tenderness and
mild right flank pain. based upon assessment the oral temperature is 39 degree Celsius. A
urinalysis showed trace blood, positive nitrites, and some bacteria. No contractions were palpated and
on vaginal examination a closed uneffaced cervical os

ACTION:
Monitor the patient’s vital signs
Advice patient to drink plenty of water
Urinate as soon as they have to urge and empty bladder completely before bed
Cephalexin 500 mg qid for one week
Cefazolin 2 g IV every 6 hour
Gentamicin 120 mg IV every 8 hours
Maintain an acidic environment of the bladder by the use of agents such as Vit.C,
Mandelamine (a urinary antiseptic) when appropriate.

RESPONSE:
Recheck the body temperature which is 36 degree celcius. The patient verbalize “okay na po
ang pakiramdam ko and hindi na masakit tuwing ako ay iihi.
